2102467 Primary Facility 2012-09-24 Routine/Complete Inspection No Violations Noted Arrived at the site at 12:05 in response to a spill of (10) gallons of radiator fluid from a GDS vacuum truck. Loren Salsman, site foreman of Comtech Industries Incorporated, accompanied me during the inspection. He informed me that the spill took place at the Hibbard Flowback site, not on the Hibbard 2H and 4H Well Pad. The drilling mud treatment system continues to operate at this site. It has been in operation for over (1) year. The plan is to move the centrifuge and associated equipment as part of the treatment system to the Hibbard Flowback Site, which is located on the former Hibbard Tank Pad. Cabot will extend the Tank Pad to accomodate the drilling mud treatment system. Cabot is in the process of obtaining a General Permit WMGR 123 for Processing and Beneficial use of Gas Well Wastewater from Hydraulic Extraction of Natural Gas for the Hibbard Flowback site.
2155929 Primary Facility 2013-04-10 Administrative/File Review Violation(s) Noted A fork truck punctured a tote containing 200 gallons of ferric sulfate. All of the fluid was released to the containment. The fluid was then removed by a vacuum truck. The operator began to remove the felt over the liner for disposal. At this time it was noticed that there was a breach in the containment. At the time of the notification a 9’X12’ area was determined to be impacted. A 20’X30’ area was excavated and the surface of this area was scraped down between 2”-4” with a backhoe. It was determined that the tote held (250) gallons and that an approximate 10-20 gallons were remaining below the puncture hole. The Department requested that a copy of the MSDS for KR-B6001 be submitted.

2491176 Primary Facility 2016-06-08 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:00 a.m., the cellar was about three quarters full of gravel with no gas detected at the perimeter of the cellar, the (9x5) vent pipe was open and venting to a tank, there was 502psi on the production casing. I took a walk around the location and noticed repair work was done to the perimeter of the well site and everything looked OK.

I left the site at 9:20 a.m.

2530989 Primary Facility 2016-10-20 Site Restoration No Violations Noted Inspection of the Hibbard A&M Pad 2 in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit. EPS, Jon Sassi, was also present on this inspection.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. Post Construction Stormwater Management BMPs appear to be installed properly according to the site restoration plans. The well pad is less than or equal to the size indicated on the site restoration plans.

Based on the site inspection, I recommend this notice of termination be APPROVED.

Jon and I also conducted a general inspection of the pad. Signs and labeling appeared to be correct. The mailbox contained the required items. There were no apparent issues with containment. Conductivity readings of water on the pad were of no concern.

2531073 Primary Facility 2016-09-20 Routine/Complete Inspection No Violations Noted Met with Cabot staff on the Hibbard A&M P2 to discuss vegetation prior to Cabot's submittal of the notice of termination of the Erosion and Sedimentation permit for the pad. Turnips were planted on the cut and fill slopes of the well pad as part of the restoration seed mix as a deer feed plot, not to be harvested. I didn't consider this an obstacle to receiving an approval of the notice of termination.
2548673 Primary Facility 2016-12-05 Routine/Complete Inspection No Violations Noted On December 5, 2016 the Department, represented by Frank Hoehle was on site to conduct a Routine Complete Inspection of the Hibbard well pad.

The wells associated with this pad are as follows: 115-20149 115-20232

This pad was stable, with no erosion or sedimentation issues at this time. There was excellent vegetation all around the site, and a very nice crop of brassicas on the site as well.
