1833430 Primary Facility 2009-09-25 Follow-up Inspection No Violations Noted At time of inspection, there was no drilling activity on site. During inspection of Hubbard 5H, gas seepage was observed in the cellar. Gas bubbling was visible along the outside of the 7" casing, however, due to the amount of water present in the cellar, it could not be deteremined where the gas was leaking from.
1835392 Primary Facility 2009-10-14 Follow-up Inspection No Violations Noted At the time of inspection, there was no drilling activity on site. The inspection was completed as a follow up of a previous inspection performed on 9/25/09 where gas seepage was observed in the cellar of the well. Since the last inspection, metal plating has been installed over the cellar of Hubbard 5H. Based on a visual inspection (which was limited by the new metal plating), there was a minimal amount of water at the bottom of the cellar, and no bubbling was observed.

No standing water was present in the cellar and no visible bubbling observed. The area around the base of the well has been filled to grade with P-gravel. The vents on the 9-5/8" and 7" casings were open. The well is online. Gas readings were collected around the well head using a Model 60 gas meter. No readings were detected. There was no activity on site.

The Department was on site at the Hubbard P1 Well Site to conduct a Routine Partial Inspection. At the time of the inspection, the E&S Inspection Log indicated that Mike Brilla (TEC) conducted a Post Rain Event Inspection on 4/27/16. The Department noted that the site appears to meet temporary stabilization requirements.

The site was constructed according to plan, and there are existing pits located on site. The vegetation was perennial, at or above the 70% vegetative standard and there were no erosional actions occuring at this time. All BMPs have been removed. I recommend APPROVAL of this Notice of Termination.
