1845000 Primary Facility 2009-10-19 Follow-up Inspection No Violations Noted At the time of inspection there was no activity on site. The well has not been fracced yet. There is no evidence of gas bubbling in the cellar of the well.
1853819 Primary Facility 2010-01-05 Routine/Complete Inspection No Violations Noted I arrived on site at 11:30. I met with Stephen Barrett and Mike Bail of Cabot. Operator is in process of fraccing well. Superior is frac contractor on site. At time of inspection, fraccing temporarily shut down due to frozen equipment. Operator at stage 3 of 11. Secondary containment is in place and working on location. Operator is using various heaters during cold temps to keep pipes from freezing, including kerozene torpedo heaters. At time of inspection, there is no secondary containment being used underneath the heaters. It was discussed with operator on site to use caution when filling these heaters, and that secondary containment may be necessary in future.

2021756 Primary Facility 2011-10-12 Follow-up Inspection Violation(s) & Outstanding Violations I arrived at the site at 10:20 AM to conduct a Follow-Up Inspection regarding the observation of daylighted liner noted in five (5) areas on the surface of the well pad during the 9-1-11 inspection conducted by the Department and potentially the appearance of drilling residuals on the surface of the well pad during the 9-21-11 inspection reports. The outstanding violation of Pollution prevention measure, Section 402 Clean Streams Law can be referenced in the 9-1-11, 9-2-11, 9-21-11, 10-5-11 and 10-6-11 inspections reports. The new violations observed during the inspection are, Section 25 Pa. Code §78.56(a) and section 301 of the PA Solid Waste Management Act, 35 P.S. §6018.301, due to the compromised liner intergrity observed during the excavation of the test pits and the drilling residuals contacting the surface of the ground.
2022595 Primary Facility 2011-10-26 Follow-up Inspection Violation(s) & Outstanding Violations Dark staining and drilling residuals were observed to be contacting the surface of the ground throughout the pit and the excavation process along with compromised liner integrity. The earthen wall of the south section of the pit in the northwest corner of the well pad, revealed only small pieces of torn pit liner. The majority of the earthen wall in this section of the pit was not covered with a liner. There were no new violations observed during the inspection. Please refer to the 10-12-11 inspection report for outstanding violations.
2022693 Primary Facility 2011-10-27 Follow-up Inspection Violation(s) & Outstanding Violations A new violation was noted during the inspection. The Department has been able to determine that the waste material in the pit was not encapsulated within the liner in accordance with 25 PA code § 78.62(a) based on the extent of the excavation. Section 25 PA code § 78.62(a)15) states "The liner shall be folded over, or an additional liner shall be added, to completely cover the waste and the waste is shaped so that water does not infiltrate the liner and is not confined above the liner". Departed from the site at 11:30 AM.
2022719 Primary Facility 2011-11-29 Administrative/File Review Violation(s) Noted Mr. Mehalko informed me in a phone conversation that sampling results revealed high levels of strontium in a section of the pit. Cabot will proceed to the remove pit material in this particualr area in the near future. A date has not been set as of yet for this event to occur. He also informed me that currently a liner is in place in the pit to prevent water from precipation events entering the pit and making contact with the surface of the ground. Upon removal of the pit material where the elevated strontium levels were detected from the sampling results, URS will perform another sampling event in attempt to achieve passing results. If passing sampling results are established, the pit liner will be removed and the pit will be backfilled with clean fill material stockpiled on the northeast corner of the well pad.
2023226 Primary Facility 2011-10-13 Follow-up Inspection Outstanding Violations - No Viols Req'd A total of 19 test pits were excavated at this site to determine the extent of drilling residuals in the pit and potential contact with the surface of the ground, liner integrity and placement and whether or not the pit underwent proper encapsulation procedures in accordance with 25 PA code § 78.62(a). I was informed that there are two (2) pits located at this site. The pits are located on the northern edge of the well pad. There is a dividing earthen berm separating the pits. The encapsulated liner is located in the pit east of the dividing earthen berm on the northeast corner of the well pad. This pit will not be excavated and pit material will not be removed due to the encapsualted liner. There is no encapsulated liner west of the dividing berm. Andy Mehalko of Cabot contacted me infroming me that Cabot will remove the entire pit material in the section of the pit not encapsulated and properly dispose of this material off-site. No new violations noted. Please refer to the 10-12-11 inspection report for all outstanding violations.
2023228 Primary Facility 2011-10-28 Follow-up Inspection Outstanding Violations - No Viols Req'd Arrived at the site at 10:30 AM. I was accompanied by Briana Betress, DEP WQS. Charles Minturn of URS was on-site during the inspection. GDS personnel on site included George Bishop and Chris Black. Up to this point still at close to 60 tons of waste matetial removed from the pit. The GDS vacuum truck remained on-site removing fluid from the bottom surface of the pit. Due to landfill permit issues, and possibly rail car arrangement, Cabot is not disposing any pit material off-site, today, 10-28-11. Removal process has been placed on hold for the time being. Cabot is in preparation for an approcahing snowstorm and will cover the pit as per Department request. We departed from the site at 15:30 and observed a cover for the pit in the back of truck as Cabot was preparing to place the cover over the pit. There were no new violations observed. Please refer to the 10-12-11, 10-26-11 and 10-27-11 inspection reports for all outstanding violations.
2024705 Primary Facility 2011-10-31 Follow-up Inspection Outstanding Violations - No Viols Req'd Arrived at the site at 10:00. I was accompanied by Briana Betress, DEP WQS. The pit was covered with a clear liner of plastic material. The GDS vaccuum truck was on-site pulling fluid off the liner covering the bottom surface of the pit. There was no other activity observed at the site during the inspection. Excavation and removal procedures of the waste material in the pit are still on hold due to landfill permitting issues and rail car arrangements. Departed from the site at 11:00.
2347640 Primary Facility 2015-02-25 Routine/Partial Inspection No Violations Noted The Department was on site at the R Hull Well Site located at 1197 Phillips Road Springville Township, to conduct a Routine Partial Inspection. During the Department’s inspection, the E&S Plan was on site and the E&S Inspection log showed that Sam Wiernusz (TEC) conducted a weekly inspection on 2/25/15. At the time of the inspection, the site was snow covered. No obvious E&S issues were observed.
