According to Company Man, open-hole logs were run in the hole. The drill string was being tripped out at the time of inspection. The Company Man indicated they will likely start running 5-1/2 casing to TD sometime tomorrow.

This is a violation of 25 PA Code 78.54 General Requirements, which provides, in part: "The well operator shall control and dispose of fluids, residual waste and drill cuttings, including tophole water, brines, drilling fluids, drilling muds, stimulation fluids, well servicing fluids, oil, production fluids and drill cuttings in a manner that prevents pollution of the waters of this Commonwealth..."

On site to evaluate a self reported spill of fluid. The operator has reported that approximately 1 bbl of brackish fluid escaped secondary containment and impacted the ground. The fluid was excavated by the time I reached the site. It was recommend that the operator use the Act 2 attainment standards for clean-up. No E/S problems were noted while on site. 2021729 Primary Facility 2011-10-12 Follow-up Inspection Violation(s) & Outstanding Violations I arrived at the site at 10:20 AM to conduct a Follow-Up Inspection regarding the observation of daylighted liner noted in five (5) areas on the surface of the well pad during the 9-1-11 inspection conducted bty the Department and potentially the apperance of drilling residuals on the surface of the well pad during the 9-21-11 inspection. The outstanding violation of Pollution prevention measure, Section 402 Clean Streams Law can be referenced in the 9-1-11, 9-2-11, 9-21-11, 10-5-11 and 10-6-11 inspection reports. The new violations observed during the inspection are, Section 25 Pa. Code §78.56(a) and section 301 of the PA Solid Waste Management Act, 35 P.S. §6018.301, due to the compromised liner intergrity observed during the excavation of the test pits and the drilling residuals contacting the surface of the ground.
