1803454 Primary Facility 2009-06-03 Drilling/Alteration No Violations Noted E/S Plan on site, Site ID sign on site. All E/S controls in place and appear to be working. The large pit had approximately 4' of freeboard during inspection. Drillers log was on site and filled out properly. According the company man, they had drilled to TD of 7120'. Upon trying to trip out of the hole, the drill pipe got stuck due to the hole collapsing in near TD. The company man indicated they eventually were able to free the pipe and trip out of the hole. At the time of inspection, the crew was circulating mud in the hole with the use of a Superior pump truck in an effort to clean out the hole prior to setting the 4-1/2" casing. I indicated to the company man to watch the freeboard in the pit they were using to circulate mud as it was getting close to freeboard limits. I also observed a skid containing several 55 gallon drums was full of rainwater. I suggested to the company man to have that pumped out to prevent a future spill.

The freeboard in the earth pit was approximately 3 - 6" from the top of the pit. It appears that some of the drilling mud had spilled out of the end of the pit onto the pad as evident from a large puddle of mud.
1804893 Primary Facility 2009-06-12 Follow-up Inspection Outstanding Violations - Viols Req'd Arrived on Site 8:00 am to complete follow up inspection on earthpit. I had spoken with Safety and Environmental officer for GasSearch on 06.11.09 about the freeboard issue at the site. I also spoke with one of Cabot's Company Man regarding the issue. A call was made at that time by the Co. man to the trucking coordinator to arrange to have the liquid removed from the pits at the Site. Upon arrival this morning to site, it appeared that one end of the earth pit has been pumped down approximately 12", however, freeboard is still above 2' limit. The other end of earthpit has between 3 - 6" of freeboard.

The pit has been relined with a new liner. A shallower pit has been installed adjacent the new reserve pit which will be the location for the shaker unit. This pit has also been lined with a new liner.

2121471 Primary Facility 2012-11-08 Incident- Response to Accident or Event No Violations Noted The Department was on site in response to a self-reported release. At the time of the inspection, the Department met with two Nahbors employees who were unaware of the release. The Department requested that they contact a Cabot employee concerning the inspection. The Department then met with Rafael Rodriguez (Completions Consultant- Cabot Oil & Gas). At this time Rafael explained that on 11/7/12 at 21:30 that a low pressure suction manifold on a pump truck came apart. It was reported that 20 bbl. were released to containment. It was initially determined that the release was “slick water” or freshwater mixed with friction reducer. This was noted to have occurred during Stage 12 pad. They are currently on Stage 15 of 17.Brody Webster of Cabot Oil & Gas arrived on site.Brody stated that they used a 100 bbl. vacuum truck to remove the liquid. A swath of sand was noted alongside the truck.

2139928 Primary Facility 2013-02-14 Follow-up Inspection No Violations Noted On site to collect pressure reading and isotopic gas sample from annulus of well. DEP reps on site = Steve Watson, Eric Rooney, and Gene Rickard. DEP met John Smelko of Cabot on site. Selman reps on site to assist with collecting gas readings and to split gas samples. Cabot shut in annulus on well @ 9am this morning. A digital gauge was installed on 7 5/8" x 4 1/2" annulus. Pressure indicated 0 psi. Gauge was removed and open vent flow was screened using Model 60 gas meter. Reading was 4% combustible gas. DEP and Cabot collected samples for isotopic analysis. The pressure gauge was then installed on the 9 5/8" x 7" annulus. Pressure reading was 0 psi. Model 60 reading was 1.5% combustible gas. DEP and Cabot collected samples for isotopic analysis. After competion of sampling, Cabot re-connected vent lines and locked vents open.
2344215 Primary Facility 2015-02-11 Complaint Inspection No Violations Noted The Department (Scott Kuchta-DEP WQS and I) was on site at the Gesford 4 & 8 well site in response to a complaint (2/7/15) concerning venting from the production tanks. At the time of the inspection, no venting was noted. The Department inspected the snow covered area around the tanks to check for indications of a fluid release. No evidence of released fluid was noted.
2509186 Primary Facility 2016-08-04 Complaint Inspection No Violations Noted At time of inspection, arrived on site at 12:05p.m. pertaining to a complaint called into the department about a backhoe and a dumpster being hauled onto the site and unknown activity. There was no activity on this site at time of inspection. There was no backhoe on site but there were two role off containers on the Castello J Pad 1 site. The marks on the entrance road indicated that the containers have recently been dropped off.

I left the site at 12:10p.m.

2642900 Primary Facility 2017-10-02 Site Restoration No Violations Noted I inspected the Gesford K, P4 site on October 2, 2017 to conduct a Notice of Termination (NOT) inspection.

The site was compact and well maintained. The access road was solid and stable. The pad surface was solid as well. The cut and fill slopes exceeded the 70% vegetative standard. On the east side of the pad, active agriculture was ongoing (field corn). There was no erosional issues apparent at the time of inspection.

I recommend APPROVAL of this NOT.

There were no violations noted.

I was accompanied by Matt Shope, DEP O&G WQS. We met with Jordan Hoffman, lease operator of Cabot. The weather conditions were partly cloudy skies with a light breeze, occasional wind gusts, and temperatures in the mid 30's. I was first informed of the release on 12-19-17. I contacted Steve Watson, Environmental Health Specialist of Cabot Oil & Gas on 12-19-17 at 13:29 to acquire the specifics of the spill incident. He explained the following details regarding the spill. "The spill occurred at the Gesford 4 pad on State Route 3023 in Dimock. The above ground dump line froze as part of the Gesford 8H well. A flange fitting on the line separated spilling production fluid to the pad surface around 8:30 PM on 12/14/17. I reported the spill to the PADEP Emergency Response Team in NERO at 9:20 PM on 12/14/17. Soap sticks were added into the well on 12/13/17 to help lift the water in the gas since it is an old
