DEP reps on site were Steve Watson, Eric Rooney, and Gene Rickard. DEP met with Mr. John Smelko of Cabot. Selman reps on site to assist in gas readings and split gas samples. A digital pressure gauge was installed by Cabot on 8 5/8 x 5 1/2" annulus. Pressure indicated was 0 psi. Gauge was removed and open vent flow was screened using Model 60. Reading was 3% combustible gas. Both DEP and Cabot collected gas samples for isotopic analysis. After completion of sampling, Cabot re-connected vent line and locked vent open.
2319458 Primary Facility 2014-11-05 Routine/Complete Inspection No Violations Noted At time of inspection, arrived at site at: 12:00 hrs. Onsite to perform routine inspections of wells / wellsites and document signage present at both Costello well pads. (Pad #1 and 2) On the Costello Pad #2: The 2V well is only well drilled on pad and was in production at time of inspection. No pressure gauges were accessible at wellhead or GPU at time of inspection. 2V well had (2) annular vents present and labeled, (8 5/8" x 5 1/2") and (5 1/2" x 2 3/8"). Both vents were open and venting to onsite poly vent tank. LEL check at vent tank opening showed 0 % LEL. Well cellar was full of gravel at time of insp. No obvious concerns were noted at wellhead or wellsite. Well tags and Entry signage were in place and up to date for both pads.

I took a walk around the location and everything looked OK.

I left the site at 9:15a.m.

2509182 Primary Facility 2016-08-04 Complaint Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m. pertaining to a complaint called into the department about a backhoe and a dumpster being hauled onto the site and unknown activity. There was no activity on this site at time of inspection. There was no backhoe on site but there were two role off containers on the Castello J Pad 1 site. The marks on the entrance road indicated that the containers have recently been dropped off.

I left the site at 12:50p.m.

2638662 Primary Facility 2017-09-21 Routine/Complete Inspection No Violations Noted Arrived at site at 11:00. Weather: sunny, warm ~ 80 degs. Onsite in response to area resident report of heavy activity occurring on pad. Upon arrival I spoke with Cabots electrical contractor in supply trailer. He confirmed that over the last week or so Cabot had been updating the wells remote data capture devices and sending unit. He stated that these updates have been completed and he will be moving the supplies trailer to next location today. Further observations of location showed that a new containment structure has been installed beneath a newly installed production tank. Old production tank was at side of location and will be removed shortly. Several new precast concrete barriers have been installed to protect newly plumbed 2" production water piping ran to new production tank.

Inspection of the Costello 2V well showed that well was lined up for automated production. Well was not producing at time of inspection. Annular vents for the (8 5/8"x 5 1/2") and (5 1/2"x 2 3/8") annuli were open and venting,

2749438 Primary Facility 2018-07-05 Complaint Inspection No Violations Noted - Arrived on location at: 08:45. Onsite in response to complaint #335360 received yesterday 7/4/18 at ~ 4:58pm. Complainant stated: GDS Vac truck labeled "Residual Waste" just drove onto Costello #2 Pad. Truck entered full and left empty." Complainant was concerned that illegal dumping or spill had occurred.

- Upon my arrival, I took note and pictures of truck tire tracks which indicated previous truck activity, pulling onto pad and backing up to access wells Production tank and secondary containment around Production tank. (See attached pics) At time of inspection the liquid level in Production tank was approx. 1/4 full. The water level in secondary containment area was ~ 6" deep. I thoroughly checked banks along the edge of pad, along entire length of access road to Costello #2 Pad and wooded area along low side of access road for any evidence of an illegal discharge or release run off. No signs of a recent release were noted at any areas checked. - Prior to leaving pad, a GasSearch Drilling Services "GDS"
