1876535 Primary Facility 2010-04-13 Follow-up Inspection No Violations Noted I inspected the well head for evidence of leakage. There was no standing water present. The base of the wellhead was filled in with P-gravel to ground level. Water was poured around the base of the wellhead where the top of the 9-5/8"casing was visible. No bubbling was observed. The 7" casing is currently vented. The well is online and producing. Gas readings were collected using a Model 60 gas meter. No readings were detected. There was no activity on site. The site has been restored.
2123786 Primary Facility 2012-12-12 Complaint Inspection No Violations Noted Arrived at the site at 11:00 in response to an alleged complaint of leaking brine tanks. The Department received the complaint on 12-11-12 at 10:35. I was accompanied by Briana Cunningham, DEP WQS. During the inspection the Department did not observe any evidence of leaking brine tanks or brine spills at the site. I used an an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen areas of ponded water inside of the earthen containment berm where the production fluid tanks are stored at the site. The highest conductivity reading I recorded was (114) microSiemens/cm. On average, conductivity readings were in the the range of (90-100) microSiemems/cm. The water inside the berm was noted to be mostly frozen and experienced little to no turbidity. Several times, I had to break the ice to record a reading. Departed from the site at 11:20.

2490173 Primary Facility 2016-06-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:20 p.m., the cellar was full of gravel with water setting on the top with no bubbling noticed, the (9x7) vent pipe was open and venting to a tank, the (7x4) vent pipe was open and venting to a tank, there was 627psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 1:15 p.m.

2629138 Primary Facility 2017-08-22 Routine/Complete Inspection No Violations Noted Arrived at location at: 09:00. There are (2) wells currently on this pad, the Ely, K 4V and 6H wells. Upon arrival I spoke with Cabot O&G Rep., Brian Pert, a Meter Tech Trainee. Brian informed that He and other Meter Techs. were in the process of retrofitting all the existing producing wells on all (3) of the Ely, K Pads (1, 2 & 3) with updated metering and Pad layouts. This work should be completed on all (3) Pads in next few weeks.
 Upon inspection of the Ely 4V and 6 H wells, both wells were set-up for automated production. Neither wells were producing at time of inspection. Both wells annuli, the (9 5/8"x 7") & (7"x 4 1/2") were open and venting to a small, shared vent tank via a common vent pipe. A gas check performed at the open top of vent tank showing 4% methane detected. A follow-up inspection will be arranged with Cabot to separate vent lines and perform individual annuli gas checks to confirm source of venting gas.

2632364 Primary Facility 2017-08-29 Routine/Complete Inspection No Violations Noted On location to perform gas checks at the Ely 4V and 6H wells annuli prior to starting a 48 Hr PBUT on these two wells. The 4V and 6H wells (9 5/8"x 7") and (7"x 4 1/2") have been separated and individually vented to onsite vent tanks. (2) Cabot Field techs and (2) Selman and Associates techs were onsite to initiate 48 hr. PBUT. At ~ 09:15 initial gas checks were performed. The Depts gas checks were performed using an Altair 5X gas meter calibrated on 8/17/17. The Depts initial gas checks showed:

- Ely 4V: (9x7)= 0% gas, Peak 10% LEL diminishing to 0% LEL; (7x4)= 100% gas, 100% LEL. - Ely 6H: (9x7)= 0% gas, Peak 20% LEL diminishing to 0% LEL; (7x4)= 0% gas, 0% LEL.

  Cabot Field techs shut both wells (9x7) and (7x4) annuli in at ~ 09:30. PBUT pressure will be recorded through test period via Total Flow Control Unit. Both wells will remain lined up for automated production throughout test period.
  We will return to conclude PBUT on 8/31/17 @ ~ 10:00.
2727098 Primary Facility 2018-05-04 Routine/Complete Inspection No Violations Noted Arrived on location at 09:30. Weather: Light rain, mild ~ 65 degs. Onsite to witness the start of 72 hr. PBUT to be performed on the Ely 6H well. Upon arrival, Both wells, the Ely 4H & 6H wells were in production. Both wells (9 5/8"x 7") & (7"x 4 1/2") annular vents were open and venting to separate poly vent tanks. Gas checks performed at both wells vent tanks showed:

- 4H: (9x7) = 0% LEL, 0% CH4; (7x4) = 4% LEL, 0% CH4 - 6H: (9x7) = 0% LEL, 0% CH4; (7x4) = 100% LEL, 100% CH4 Both wells Prod. Casing, Static and Differential pressure were being monitored by Cygnet monitoring system. Each wells pressures were collected off LED read outs by gas production units "GPU". Both wells pressures are listed below: (Prod. Casing Press. = "PCP"; Static Press. = "SP"; Differential Press. = "DP") - 4H: PCP = 484.4 psi.; SP = 491.3 psi.; DP = 69.8 psi. - 6H: PCP = 470.2 psi.; SP = 494.6 psi.; DP = 147.2 psi. Cabot O&G Reps. Jeremy Rapp and Steven Catalfamo arrived onsite at ~ 09:50. The 6Hs annuli vents were shut in at 10:

2729749 Primary Facility 2018-05-07 Routine/Complete Inspection No Violations Noted Arrived on location at 09:30. Weather: Overcast, mild ~ 55degs. Onsite to conclude the 72 hr. Pressure build-up test ¿PBUT¿ being performed on the Ely 6H well. This wells (9 5/8"x 7") & (7"x 4 1/2") were SI on 5/4/18 @ 10:00. Upon arrival, I met with Cabot O&G Reps., Jeremy Rapp and Steven Catalfamo; Selma & Assoc. Rep., Matt Went; D.E.P. Geologist, Eric Rooney; D.E.P. WQS Supervisor, Brianna Cunningham and WQS, Casey Baldwin.

- At 10:00 the ending pressures were checked with digital gauge and recorded. (9x7) = 0.0 psi.; (7x4) = 84.4 psi. Gas checks performed at each annuli showed: (9x7) = 0% LEL, 0% CH4; (7x4) = 100% LEL, 100% CH4. Gas samples were collected off the (7x4) annulus by the Dept. and Selma & Assoc. at ~ 10:00. Samples will be sent out for laboratory analysis. At conclusion of sampling, both these annuli were opened up to vent to separate poly vent tanks.

Both wells, the 4H & 6H were producing at time of insp. Prod. Casing, Static and Differential pressure were being monitored by Cygnet monitor
