2065320 Primary Facility 2012-05-03 Follow-up Inspection No Violations Noted Follow up on report of well "spewing". At time of inspection Cabot crew on site repairing production unit. According to Cabot, a gauge for dump mechanism inside production unit wore out due to sand flow. As a result water was dumping directly to holding tanks in a manner that created a loud hissing noise to be heard. No spills were reported or observed.
2247507 Primary Facility 2014-02-20 Complaint Inspection Pending I traveled to the Lewis 2 Well Site (115-20030) in response to a complaint of a commotion on the well site. Mr. Smales(well tender) explained the following: While Mr. Depue was plowing snow on 2/19/14 at 12:00 PM, he backed up and hit the Lewis 2 well head. At that time, gas began to leak from a threaded fitting with a valve causing gas to escape. Crews were working on site from 12PM-9PM. It was reported that no fluid was lost but just a hissing sound was heard escaping from the well. EPS was called in and they installed a choke manifold and vented gas to the production tank. It was reported that they put a hydraulic line on top of the well head to pump 55 bbl of “brine” transported from Comtech’s A&M Hibbard Water Treatment Facility on Rte. 29 into the Lewis 2 well to kill the well (reported as successful). Cabot reported that it took 4 hours to complete this. This was done because the valve was below the shut off on the well head.Diversified Security was on site last night.

2398204 Primary Facility 2015-08-13 Routine/Complete Inspection No Violations Noted
2497393 Primary Facility 2016-06-28 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m., the cellar was full of gravel with no gas detected, the (7x4) vent pipe was open and venting to a tank, there was 604psi on the production casing and 515psi on the production tubing. I took a walk around the location and everything looked OK.

I left the site at 13:50p.m.
