No readings were detected. There was no activity on the site at the time of inspection, and the site has been restored.

Will be using 350 +/- sks Class A w/ gasblock - Superior. Met with Johnny Crawford - Company Man on site. he provided a copy of the workover prognosis to DEP at time of inpsection.

Will be running Bond log to locate TOC on back side of 4" prod. casing. Pumped 89 bbls of 15.6 lb Superior Standard A cement at 2 bbls/min. Displaced with 64 bbls freshwater. Had 15 bbls returns and left 500 lbs on hole overnight. TOC cement should have been tagged above retainer set at 1600', but no cement tagged. Likely fell back through perfs above retainer into backside of 4". Had 0psi on gauges this morning. DEP will follow up.
1944075 Primary Facility 2011-01-20 Drilling/Alteration No Violations Noted Johnny Crawford - Company Man. Stone Service Rig over hole. Perf'd and sqeezed cement on backside of 4 1/2" prod. string. Tested casing - to 1500 lbs. Lost 500 lbs over 15 mins and 45 mins later held at 1000 lbs. Setting packer at 1570 and testing backside again to isolate leak. Likely stil leaking through perforations. Planning to pump additional cement into hole. DEP will be notified when test complete.

Testing tailpipe. if test good then Packer will be set at 1723'. patch set on 4 1/2" @ 1569 - 1599'. Packer length - @4'. Tubing is plugged - perforating bottom joint then swabbing hole to bring back on line. No gas shows while serviceing well.

2123784 Primary Facility 2012-12-12 Complaint Inspection No Violations Noted Arrived at the site at 11:25 in response to an alleged complaint of leaking brine tanks. The Department received the complaint on 12-11-12 at 10:35. I was accompanied by Briana Cunningham, DEP WQS. During the inspection the Department did not observe any evidence of leaking brine tanks or brine spills at the site. I used an an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen areas of ponded water both inside and outside of the earthen containment berm where the production fluid tanks are stored at the site. Inside the berm, I recorded a conductivity reading of (600) microSiemens/cm. Outside the berm the meter revealed a reading of 128 microSiemens/cm. The water was noted to be mostly frozen and experienced little to no turbidity. I had to break the ice to record a reading. Departed from the site at 11:40.

2490176 Primary Facility 2016-06-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:20 p.m., the cellar was full of gravel with no gas detected at the perimeter of the cellar, the (9x7) vent pipe was open and venting to a tank, the (7x4) vent pipe was open and venting to a tank, the (4x2) vent pipe was open and venting to a tank, there was 547psi on the production tubing. I took a walk around the location and everything looked OK.

I left the site at 1:15 p.m.

2551744 Primary Facility 2017-01-09 Routine/Complete Inspection No Violations Noted The department was on site to conduct a Routine Complete inspection of the Ely K, 2V Well Pad. The well associated with this pad is the 115-20015.

The pad was small and well vegetated with no erosional events occurring at this time. There was some small areas of stone stacking occurring outside the LOD, but no impact to the site or access road was observed. The surface was loose and porous.

2642559 Primary Facility 2017-10-02 Site Restoration No Violations Noted I inspected the site on October 2, 2017 for Notice of Termination (NOT). The site was situated at the end of a long access road. It appeared there was ongoing maintenance of the onsite equipment (painting)The access road was in good shape, solid and pretty well graded. The pad surface was in good shape. The site as a whole was small and well vegetated on the cut and fill slope, in excess of the 70% vegetative standard. The area around the site was comprised of dense hardwoods mostly. There were no erosional issues observed at the time of inspection.

I recommend APPROVAL of this NOT.

There were no noted violations.
