2579427 | Primary Facility | 2017-04-05 | Routine/Complete Inspection | No Violations Noted | Inspection to assess site conditions after spudding operations. Gravel has been placed back onto the well pad. No evidence of accelerated erosion lr sedimentation. No violations cited. |
2585887 | Primary Facility | 2017-04-18 | Drilling/Alteration | No Violations Noted | Arrived @ 12:30. Weather sunny. Patterson 569 rig move to the Nelson 2H was not yet completed at the time of my inspection.
Left @ 12:45. |
2588624 | Primary Facility | 2017-04-25 | Drilling/Alteration | No Violations Noted | Arrived @ 12:20. Weather: cool, cloudy showers. Patterson 569 at 6460’ at the time of my inspection (kick off point at 6950). Met with Company Man Jeff Browning. Reviewed BOP tests (4/20/17 and 4/21/17), and cement reports. Left location @ 13:15. |
2590312 | Primary Facility | 2017-05-02 | Drilling/Alteration | No Violations Noted | Arrived @ 13.15. Weather: windy with showers. Patterson 569 at 539’at 6460’ at the time of my inspection. Met with Company Man Jeremy Boyd. Started drilling on 5/1/17, needed to trip out for bit – otherwise going very well. Previously drilling completed on the Nelson 2H. Left location @ 14:00 |
2599790 | Primary Facility | 2017-05-31 | Routine/Complete Inspection | No Violations Noted | Arrived @ 11:30. Weather: overcast. Moran Logistics staff were on site, getting ready for a toe prep on the Nelson 2H and 3H. The purpose of this inspection was to follow-up on Peg Farrell’s 5/26/17 notification of bubbling outside of surface casing for the Nelson 3H. There are 3 wells on this location. At the time of my inspection, the Nelson 1H was producing with compression. The 2H and 3H were TD’ed but not stimulated.
Nelson 2H: The cellar was partly filled with gravel, but no water. The 13-3/8” x 9-5/8” and 9-5/8” x 5-1/2” annuli were plumbed to the same 550-gallon poly tank as the Nelson 3H. It was not possible to check the individual annuli for methane. |
2607687 | Primary Facility | 2017-06-26 | Routine/Complete Inspection | No Violations Noted | Two wells (2H and 3H) to be fracked in coming days. Baker tanks staged and within containment. Universal appears to be frack contractor. No violations noted. |

2618045 | Primary Facility | 2017-07-20 | Routine/Complete Inspection | No Violations Noted | Site was in flowback phase, with temporary separators and dehy units connected to the 2H and 3H wells. Two tenders on site with the system. Secondary containments in good condition, and all water on the pad showed low conductance (.37- .65 mS/ cm). |
2669761 | Primary Facility | 2017-12-06 | Routine/Complete Inspection | No Violations Noted | Arrived at 12:15. Weather: cold, windy. Met with Jon Hafer (Chief) to observe conclusion of 168- hour PBT and isotopic sample collection.

2752692 | Primary Facility | 2018-07-17 | Routine/Complete Inspection | No Violations Noted | Three wells staged for production phase. Three each water separators. Dump lines wrapped in insulating blanket. No leaks detected from production equipment. No violations noted. |
2784705 | Primary Facility | 2018-10-03 | Routine/Complete Inspection | No Violations Noted | Chief Oil &Gas submitted a Well Site Restoration Report (WSRR) for the Nelson pad. The WSRR was received by the Department on 9/11/18.
The pad has been reduced in size and vegetation was well established (>70% cover) at the time of inspection. All PCSM BMPs have been installed and appeared to be functioning as designed. There are 3 properly tagged Marcellus wells staged for production. There are also 3 produced water tanks, 3 GPUs, and 1 compressor onsite and in secondary containment. There was some minimal corrosion noted on the produced water tanks (Photo #12). There was also corrosion noted around a weld on pipeline leading to the GPUs (Photo #9). Near the wells there were 2 containers with hoses that were not labeled (Photo #6). A tank of used motor oil was near the compressor with a very faded hazard label(Photo #11). There was also a container in a "Tank Vault" that had no visible label (Photo #7). It is recommended that all storage containers be visibly and legibly marked with proper labels. It is |
