2573840 Primary Facility 2017-03-22 Routine/Partial Inspection No Violations Noted Inspection to assess site conditions during drilling operations. Due to snow cover, assessment of site conditions relating to erosion and sediment control could not be conducted. All chemicals and equipment were stored within containment. No violations cited during this inspection.

Numerous small spots of iridescent sheen were observed on puddles along the back (south) side of the well pad. The source of the spots is likely a piece of equipment with a small leak. Though the amount of sheen observed does not indicate that a reportable release occurred, it is recommended that all equipment be checked regularly for leaks and drips that could result in the release of fluids to the ground surface and surrounding environment. No violations were cited.

At the time of my inspection the H&P 513 was completing the lateral and was at 10,700’. Met with company man Brandon Jesson who said the drilling was going well. Walked the site with the company man. The small location appeared to be well organized. Left 14:35.

No violations enforced at this time.
2608448 Primary Facility 2017-06-15 Follow-up Inspection No Violations Noted Arrived at 10:40. Weather: cloudy, cooler. There were three wells at this location: The Hurley 1H, a producing well was shut-in. The Hurley 2H and 3H were TD’ed. I was accompanied by Jon Hafer (Chief), who also brought a fresh water truck, and a vac truck. The purpose of this inspection was to fill the cellars of the Hurley 2H and 3H with enough water to determine whether there was methane outside of surface casing. Water was added to each cellar to a level just above the wellhead flutes, where the cement between the conductor and surface casing was exposed. Following the addition of the water, we waited approximately 15 minutes and searched for any signs of bubbling. No signs of bubbling were seen in the cellars of the Hurley 2H and Hurley 3H. Following these observations, the vac truck removed the water from the cellars. The result of this inspection for the Hurly 2H and the Hurley 3H is NOVIO, because no violations were observed during this inspection. Left location @ 11:45.
2614041 Primary Facility 2017-07-06 Routine/Complete Inspection No Violations Noted No activity on pad when I arrived. Three wells on the pad, all tagged (although the tags on the 2H and 3H wells are temporary). 3 banks of Baker tanks in containment on the pad; small amounts of water in these containments showed conductance from .03- .5 mS/cm. Two poly tanks were present on the site, hooked to the annuli of the wells. The volume of fluid in these tanks was low 2-4" in the bottom) and had a slightly briny petroleum odor. These poly tanks should likely be in secondary containment, as the substances contained in them fall under the definition of "regulated substances".

2625585 Primary Facility 2017-08-14 Routine/Complete Inspection No Violations Noted Arrived at 14:17. Weather: cloudy, humid. There were no personnel on site at the time of my inspection. There were three producing wells at this location: the Hurley Unit 1H, 2H and 3H.

Hurley 3H, cellar partly filled with gravel Top gauge: 2225 psi (0-5000 psi analog) The Hurley Unit 2H and 3H annuli were plumbed to the same poly tank. Methane was detected at the tank (30% methane/volume, Altair 5X gas meter calibrated on 8/14/17). 13-3/8” x 9-5/8”: no combustible gas detected 9-5/8” x 5-1/2”: 35% methane/volume. The result of this inspection was NOVIO, because no violations were observed at the time of the inspection. Left @ 14:35

2788125 Primary Facility 2018-10-11 Routine/Complete Inspection No Violations Noted Chief Oil & Gas submitted a site restoration extension request to the Department on 10/11/18. Arrived onsite earlier that day for a routine inspection. It had been raining throughout the day.

The pad has been partially reduced in sized and overall is >70% vegetative cover. All PCSM BMPs appeared to be installed and functioning as designed. There were some small areas lacking in vegetation but were stabilized with straw and netting. Compost filter sock (CFS) remained along the access road and surrounding the pad including the revegetated areas. At the entrance of the access road there was a low spot which was retaining water(Photo #9).

There are 3 properly tagged Marcellus wells staged for production. There are 3 GPUs and 3 produced water tanks onsite and in secondary containment. The thief hatch on the 1H tank appeared to be open at the time of inspection (Photo #6). All secondary containment appeared to be in good condition.

2796336 Primary Facility 2018-10-26 Routine/Complete Inspection No Violations Noted Chief Oil & Gas submitted a Notice of Termination (NOT) for the Hurley pad covered under ESX11-113-0023(1). The NOT was received by the Department on 10/24/18.

A routine inspection was recently conducted on 10/11/18, ID# 2788086. PCSM had been observed during this inspection. The site was well vegetated with the exception of some spots that were stabilized with straw and erosion control matting.

During the inspection on 10/26/18 the straw covered areas showed some new vegetation growing. The pad size was previously reduced and the site was >70% vegetative cover. No E&S issues were noted during the inspection. However, compost filter sock (CFS) remained around most of the pad and along sections of the access road. Temporary E&S controls should be removed as part of site restoration.

The Department recommends returning the NOT and resubmitting it after the CFS and any other materials not part of PCSM have been removed.

2829852 Primary Facility 2019-01-18 Routine/Complete Inspection No Violations Noted Chief Oil & Gas submitted a Notice of Termination (NOT) for the Hurley pad covered under ESX11-113-0023(1). The NOT was received by the Department on 1/17/19. Arrived onsite at 09:16; the weather was ~25°F and lightly snowing. The gate was open when I arrived.

There were 3 wells staged for production and the cellars were filled with gravel. There were 3 GPUs and produced water tanks in secondary containment. There was a single 550 gallon poly tank plumbed to the well annuli and not on additional secondary containment.

This pad was previously inspected for an NOT on 11/8/18, ID# 2800885. During this inspection it was noted that PCSM had been addressed, the site appeared stable, and the site was greater than 70% vegetative cover. The NOT was returned based on compost filter sock (CFS) remaining in place.

The CFS has since been removed and the contents spread over an area to serve as a soil amendment. The vegetation still appeared sufficient and overall the site appeared stable.
