At the time of my inspection, approx 7 joints were in with approx 29 joints to run. Drilled to 1541’ and intermediate casing shoe will be at 1519’. Left location @ 10:35.
2544454 Primary Facility 2016-12-13 Routine/Partial Inspection No Violations Noted Routine inspection to assess overall site conditions. Approximately 3-4 inches of snow cover is present over the entire site. No Chesapeake representatives were present.

An air-drill rig has just been removed from the site, and the containment for the drilling activities is in place. No evidence of spill such as staining or odors was observed. Due to cold weather and snow cover, direct observation of the entire well pad surface was not possible. Puddles on the pad surface were frozen, so no screening of electrical conductivity was completed.

No violations were cited during this inspection.

2559334 Primary Facility 2017-02-06 Routine/Complete Inspection No Violations Noted Routine inspection to assess site conditions during drilling operations. Mark Calicutt of Chief was the company man on site.

No evidence of releases such as odors, sheens, or staining was observed. Five barrels were observed being stored outside of containment. I mentioned these to Mr. Calicutt who stated they would be moved to containment immediately. All other chemicals, materials and equipment on site were being stored within containment.

No violations were issued during this inspection.

2564805 Primary Facility 2017-02-24 Routine/Complete Inspection No Violations Noted Inspection to assess site conditions after drilling operations. No violations cited.
2581997 Primary Facility 2017-04-12 Routine/Complete Inspection No Violations Noted Three wells drilled and one in production phase(2H). One separator within plastic containment. At least detecting from production equipment. The plastic liner and felt remain on location.
2784693 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine site inspection. The pad has been been reduced in size and the site exceeds 70% vegetative cover.

There are 3 properly tagged Marcellus wells staged for production. There are also 3 produced water tanks and 3 GPUs onsite and in secondary containment.

At the time of inspection it was observed that a white storage container plumbed to a well head was missing a cap (Photo #4). It is recommended that this cap be replaced to prevent rainwater from entering the container and possibly causing it to overflow.
