Met with company man Billy Swanson who was onsite to witness cementing of conductors. Other personnel onsite included staff from: Universal Well Services and Mr Dirt. A Soilmec SR-30 hydraulic rotating rig had been used to drill the cellars and conductors was still on location. At the time of my inspection Universal Well Service was cementing in the conductors for the Hart N Unit 2H and Hart S Unit 1H. Left location @ 14:14.
2428933 Primary Facility 2015-12-01 Routine/Complete Inspection No Violations Noted Routine inspection conducted to review site conditions and E&S controls. Crews were on site completing installation of the conductor casing.

The drilling rig is mostly assembled, and crews are continuing to set up appurtenant equipment. Fuel and chemicals on site are all within containment. 2433995 Primary Facility 2015-12-16 Drilling/Alteration No Violations Noted Arrived at 09:00, weather cold and cloudy. Paterson 254 on site and at 5901’ after 6 days drilling the Hart N Unit 2H well. Rig was getting ready to kick-off. Drilling was going well. Last BOP test was on 12/12/15. Took a trip around the location. There were large ruts at the front of the pad, these have been rocked. There were no violations at the time of my inspection. Left @ 09:40.
2436174 Primary Facility 2015-12-28 Drilling/Alteration No Violations Noted Arrived @ 10:50, weather cold and grey. Met with company man Paul Rico. Paterson 254 at 13,188’ MD. Should be done drilling lateral today and start casing tomorrow, according to Rico. On 12/21/15 lateral encountered an unanticipated fault that downdropped the Marcellus so that the lateral passed into the Purcell. Deepest drilled prior to the sidetrack was 12, 175’ MD. Pulled back to 9783’ MD and drilled down to conduct an open-hole sidetrack to get back into the hot Marcellus. The sidetrack went well, according to Paul Rico. Two Haliburton cement trailers were on location in preparation for the cement job. Off containment to the left of the entrance, I observed a minor hydraulic oil under a soil compactor. The oil (¿ 0.1 gallon) appeared to come from a leaking hose. There was a sheen on puddles near the compactor also. I showed this to Paul who said it would be promptly fixed. Left location @ 12:00
2467917 Primary Facility 2016-03-24 Drilling/Alteration No Violations Noted Arrived @ 9:30, weather warm. This well was TD’ed at the end of 2015 (IR #: 2436174). Met with Jon Hafer (Chief) to determine whether methane detections on 13-3/8” x 9-5/8” casing annulus were from the casing annulus or due to cross-flow contamination for the 9-5/8” x 5-1/2” annulus. Prior to the field meeting Hafer had separated the annular vents. Monitoring was conducted with an Altair 5X gas meter calibrated on 3/24/16, and a MultiRae meter conducted on 03/24/16.

13-3/8” x 9-5/8”: 8.9% methane 9-5/8” x 5-1/2” 50.6% methane. Since the operator reported this annular gas there is no violation associated with this inspection.

2499259 Primary Facility 2016-07-06 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions and containment/materials storage for well completion operations.

Temporary erosion and sediment control BMPs are in place and in good condition. The site is well vegetated with a mix of annual and perennial plant species. No evidence of accelerated erosion or sedimentation was observed.

Well completion operations are underway. All pumps, trucks, tanks, totes and other equipment were observed to be located in or over containment. No areas of apparent spilled material were observed inside or outside of the containment. Several puddles on the well pad surface, outside of containment, were screened to estimate the conductivity of the fluid. None of the screened puddles had an electrical conductance above 675 uS/cm.

The containment around the tanks holding reuse water (flowback fluid/brine) was screened; the electrical conductivity of the fluid within the containment was screened at 7.9 mS/cm. The amount of fluid present in the containment was not signif

2503466 Primary Facility 2016-07-21 Routine/Partial Inspection No Violations Noted Routine inspection to assess site conditions after well completion operations.

Much of the pad containment liner remains in place. Several puddles on the containment were screened for electrical conductivity and ranged from 2.1 - 3.0 mS/cm. One of the puddles that exhibited elevated conductivity was adjacent to two tears in the containment. Prior to removing the containment, all fluids should be removed to minimize the risk of potential contamination to the pad surface materials and ground and surface waters. Further, due to the proximity of fluids with elevated conductivity to the tears observed in the liner, the pad material beneath the tear should be screened to ensure that fluids from the containment have not contaminated the pad surface material. The Department requests that the outcome of the screening be provided once complete.

Though containment is in place for the frac tank batteries on site, one frac tank was observed to be situated outside of containment. Though this tank was apparently empty

2791577 Primary Facility 2018-10-18 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 13:09; weather was ~42 °F, sunny, breezy. The E&S permit for this site is still active and expires 9/29/2020. There were some shallow puddles on the pad.

There were 4 properly tagged Marcellus wells staged for production. Each well cellar contained water; no bubbling was observed. There were 4 GPUs and 4 produced water tanks onsite and in secondary containment. A row of temporary flowback tanks remained onsite and in secondary containment.

At the time of inspection all PCSM appeared to be installed and functioning as intended. The site was >70% vegetative cover and no E&S issues were observed. The gravel portion of the pad appeared to remain at its full size. Some compost filter sock remained onsite.

It is recommended that temporary E&S measures be removed once permanent stabilization has been achieved.
