2558090 Primary Facility 2017-01-25 Routine/Complete Inspection No Violations Noted Arrived @ 12:10. Weather: cloudy, warm, snow on location. No one was on location at the time of my inspection. There is one spudded well, the Bahl Unit 1H. Chief completed drilling on 2/6/16. Well has not yet been fracced. Location stakes were set out for the Bahl 2H and 3H. There was ice in the well cellar. The well was locked and shut-in.

5-1/2”: 1100 psi (0-10,000 psi, analog) Annular vents were plumbed to a polytank where 100% methane was detected (Altair 5X gas meter calibrated on 1/23/17). 9-5/8” x 5-1/2”: 100% methane 13-3/8” x 9-5/8”: no methane detected. Baker tanks have been removed. There were no violations at the time of my inspection. Left @ 12:20.

2572247 Primary Facility 2017-03-13 Drilling/Alteration No Violations Noted Arrived @ 13:49. Weather: cloudy, snow storm predicted. Site being prepared for drilling. Containment was down and covered with mats. At the time of my inspection an Allison Crane crew was onsite. Also onsite was a Keystone Clearwater Solutions crew. I spoke to Mark Maguire who said that they were putting OBM from the Yonkin B in the tanks on the right hand side (looking toward the road). They were putting water in the tanks at the back on the left, and base oil in the tank at the front on the left.

The Bahl Unit 1H was shut in and surrounded by fencing. Bahl 2H and Bahl 3H conductors were cut and beveled. Left @ 14:00.

2586731 Primary Facility 2017-04-25 Routine/Complete Inspection No Violations Noted Drilling completed. Well development containment removed. Rig matting stacked on side of well site. Various storage containers remain on site. No violations noted.
2608387 Primary Facility 2017-06-27 Routine/Complete Inspection No Violations Noted I arrived on-site @ 09:20 hrs. and met Justin Myers, Chief company man. Frac ended Sunday 6/25, and most equipment had already been moved to the Nelson pad for the upcoming frac there. Several banks of Baker tanks and a pair of pump trucks were still present on the site. Water puddled on the pad surface demonstrated low conductance (.3- .980 mS/ cm). Berms had been removed from pad containment for equipment removal; water puddled on the liner also mostly showed low conductance (.4- 1.3 mS/ cm); however, one area near the pump trucks had puddles ranging from 2 mS/cm to greater than 19.99 mS/ cm. The volume of these puddles were low (the largest being 2-3 gallons), and a vac truck operator was immediately deployed to the area by Mr. Myers. Overall condition of the pad liner appeared to be good. PPC and ER plan were in the site mailbox; I departed the site @ 10:15 hrs.
2658590 Primary Facility 2017-11-07 Routine/Complete Inspection No Violations Noted Arrived at 9:50. Weather: cold, cloudt, windy. Andrea Mullen (DEP geologist), Ulyana Trynovich (DEP OGI), and I met with Jon Hafer (Chief Environmental Specialist). There were 3 producing wells on the pad. The purpose of this inspection was to examine the annular plumbing on the 2H and 3H wells (which employ the GE RSH2 well head) and the 1H ( which uses the LSH well head), and participate in the acquisition of production gas isotopic samples for the 2H and 3H wells. Following the well inspections, the abandoned well was monitored for combustible gas. Combustible gas was not detected (Altair 5X gas meter calibrated on 11/7/17). During this inspection a crew was working on one of the Bahl pad sed basins.
2669336 Primary Facility 2017-12-06 Routine/Complete Inspection No Violations Noted Arrived at 9:30. Weather: cold. I met with Jon Hafer (Chief Environmental Specialist). There were 3 producing wells on the pad. The purpose of this inspection was to participate in the conclusion of the 168-hour PBT and isotope sampling collection on the Bahl 2H and 3H wells. Bahl 1H: in production. Top gauge: 1500 psi.

2H: 9-5/8" x 5-1/2": 519.75 psig, 13-3/8" x 9-5/8": -.35 psig, 20" x 13-3/8": 1.74 psig. 3H: 9-5/8" x 5-1/2": 616.8 psig, 13-3/8" x 9-5/8": 48.1 psig, 20" x 13-3/8": 5.48 psig. Overall gas levels are declining.

2695007 Primary Facility 2018-02-15 Routine/Complete Inspection No Violations Noted Arrived at 12:30. Weather warm with snow and ice melting. There are 3 wells in production on site and an abandoned well off-site, but nearby. Bahl 1H: ice in cellar, 5' below surface, no bubbles. Top gauge: 1200 psi (0-5000 psi). 5-1/2": 1225 psi (0-5000 psi). 9-5/8" x 5-1/2": 36% methane/ volume (Altair 5X gas meter calibrated 2/14/18).

Bahl 3H: cellar water-filled. Top gauge: 1200 psi, 5-1/2": 1150 psi. Bahl 2H: low level water and ice in cellar. Top gauge: 1150, Bahl 2H and 3H are plumbed to the same polytank. Tank about 1/5 full of oil-based mud mixture. 60% methane/ volume at tank mouth. Nearby abandoned well: some water in base of casing - no bubbling. No methane detected.

2725842 Primary Facility 2018-05-03 Routine/Complete Inspection No Violations Noted Three wells staged for production phase. Four each water separators. Dump lines wrapped in insulating blanket. No leaks detected from production equipment. See E&S issues. No enforcement taken at this time.
2785643 Primary Facility 2018-10-09 Routine/Complete Inspection No Violations Noted Weather was 70° and cloudy at time of inspection. There were no chief representatives on location at time of inspection. Well had a 0-5k psi gauge on the production tubing, the production casing and the pay line. Well also had a digital gauge on the production casing. The digital gauge on the production casing read 1034 psi, tubing pressure was approximately 1200 psi and pay line pressure was approximately 1050 psi at time of inspection. There were 2 annulus vents plumbed to surface. They were plumbed together and ran over to a poly vent tank. The tank had 100% LEL and 8% methane detected at time of inspection. No violations were noted at time of inspection.
2835737 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:38; the weather was ~50°F, clear, and calm. The gate was open when I arrived.

There were 3 properly tagged Marcellus wells staged for production. The well cellars were filled with water; no bubbling was observed. The wells were plumbed to 2 poly vent tanks that were not on additional secondary containment. There were 3 GPU's and produced water tanks onsite and in secondary containment.

There were 2 infiltration basins constructed at this pad. Both basins had some remaining ice in them. Some areas that were free from snow were well vegetated. Sections of filter sock remained in place around the site. The last logged E&S inspection was 1/14/19.

The Department recommends maintaining a proper inspection schedule and noting those inspections in the onsite log.

No violations noted.
