2498150 Primary Facility 2016-06-30 Routine/Complete Inspection Pending : Arrived @ 10:10. Weather: sunny, warm. No company representatives were present during the inspection. There were 5 wells on the site. There was no GPU or produced water tanks. There were two annular vent pipes for each well. These were open and venting to three 500-gallon polytanks.

Kuziak B 6H. Cellar full of cloudy fluid. 13-3/8” x 9-5/8”: 5% methane 9-5/8” x 5-1/2”: 100% methane. Both annular vents share the same line to the polytank. Asked Jon Hafer (Chief) to isolate the annuli. The IR result is pending a follow-up inspection after the annuli have been isolated. Left location @ 11:10.

2509988 Primary Facility 2016-08-04 Follow-up Inspection No Violations Noted Arrived @ 10:10. Weather: sunny, warm. No company representatives were present during the inspection. There were 5 wells on the site. There were no GPU or produced water tanks. There were two annular vent pipes for each well. This follow-up inspection was prompted by an Inactive Well Status application for Kuziak Unit B 3H. Previously on 6/30/16, combustible gas was detected on the Kuziak Unit B 5H and 6H wells at the 13-3/8” x 9-5/8” annulus. Typically, the 1” , 13-3/8” x 9-5/8” annular vent T’s into the 2” 13-3/8” x 9-5/8” annular vent, allowing for cross-contamination. To eliminate that possibility in this follow-up inspection, the annuli were open and isolated from each other prior to my arrival on site. Monitoring was conducted with Altair 5X gas meter (calibrated: 7/26/16).

2516505 Primary Facility 2016-08-31 Follow-up Inspection No Violations Noted Arrived @ 13:00. Weather: sunny, warm. Met with Jon Hafer and Peg Farrell (Chief). The purpose of this inspection was to evaluate flow on the Kuziak B 6H and 4H wells and begin PBTs, as a follow-up to previous Inactive Well Status Application inspections for top-hole only wells on this location: Kuziak Unit B 2H, 4H, 5H and 6H wells.

The Kuziak Unit B 3H is TD’ed. Kuziak B 6H: 9-5/8” casing flow: 6180 ft3/day, (differential pressure reading across a 3/8” orifice plate restriction). Kuziak B 4H: 9-5/8” casing flow: 5.28 ft3/day. Jon Hafer set up pressure transducers for the PBT testing and shut-in the annuli. At approximately 14:32 bubbles began to appear in the cellar of the Kuziak B 6H. We agreed to meet back at the site at 09:00 the following day and vac out the cellar to determine the source of the bubbling. Left location @ 14:45

2516636 Primary Facility 2016-09-01 Follow-up Inspection No Violations Noted Arrived @ 08:53. Weather: breezy. Met with Jon Hafer and Peg Farrell (Chief). Vac truck onsite. The purpose of this inspection was to determine the source of the bubbles that became apparent after the 9-5/8” casing vent was shut-in yesterday (IR #). The bubbling today was much more vigorous than the initial bubbling on the previous day. Slowly vaccing the water out of the cellar revealed the source of the leak to be the body lubrication fitting for the hanger seals. Prior to blowing down the 9-5/8” casing to investigate this further, the pressure transducers were checked (approximately 19 hours after shut-in).

9-5/8” casing: transducer reading: 60 psi absolute, corrected for atmospheric pressure: ¿ 45 psi, 13-3/8” x 9-5/8”: 12 psig – combustible gas was not been detected on this annulus previously, suggesting that the pressure build is a manifestation of leaking hanger seals in the well head. 2531971 Primary Facility 2016-10-07 Follow-up Inspection No Violations Noted Arrived 11:20 Weather: sunny. Met with company man Floyd Roberts and environmental compliance specialist Jon Hafer (Chief). Workover rig and drilling mud management equipment onsite. To prepare the topholed Kuziak B 2H, 4H, 5H, and 6H wells for Inactive Status, the plan was to remove SOBM, clean and repurpose the mud when possible, and then set balanced cement plugs in the topholed well. During discussions on the pad it emerged that 1ppg brine was being used in between the cement plugs. At the time of my inspection operations were completed on the 2H, 4H, and 5H, and starting on the 6H. Mud in the 5H and 6H mud had separated out to base oil and weighting agent. Fluid level was also down on the 6H. Plan to conduct a follow-up inspection to determine whether the vertical flow of gas has been stopped. No violations were noted during my inspection. 2532002 Primary Facility 2016-10-24 Follow-up Inspection No Violations Noted Arrived 12:00 Weather: cold, cloudy, windy. Met with environmental compliance specialist Jon Hafer (Chief) for this follow-up inspection at the completion of a 72-hour PBT for the 4H, 5H, and 6H wells. The PBT for the 2H was completed on 10/10/16. There was a frac head on the 3H which had been toe prepped. A DFIT test was underway. For each well, there was no pressure build on the 9-5/8” casing and on the 13-3/8” x 9-5/8” annulus. No violations were noted at the time of this inspection. Left location @ 13:17.
2553036 Primary Facility 2017-01-11 Routine/Complete Inspection No Violations Noted Routine inspection to assess site conditions after fracing operations. No Chief representatives were present during the inspection.

Containment was still in place over a large area of the site. Approximately 1/2" of fresh snow cover was present. No evidence of any spills or relases, such as staining, sheens, or odors, was observed both on and off containment. Puddles that were present on the site were frozen, and thus not screened for electrical conductivity.

A request for a 2-year extension of time for site restoration was received by the Department of Environmental Protection on April 07, 2016. To date, that extension request has not been processed.

No violations were cited during this inspection.

2628072 Primary Facility 2017-08-16 Routine/Complete Inspection No Violations Noted Arrived @ 10:30. Weather: hot. There were no personnel onsite at the time of my inspection. 5 wells on site. The Kuziak B 3H was in production. Kuziak B 2H, 4H, 5H, and 6H had Regulatory Inactive Status and were top-holed, and temporarily plugged.

Kuziak B 2H: night cap, annuli plumbed to 500 gallon polytank, no methane detected, very little fluid in tank. Cellar full, no bubbles. Kuziak B 3H: in production, cellar water-filled, no bubbles.
