2385638 Primary Facility 2015-07-07 Routine/Complete Inspection No Violations Noted Arrived @ 13:24 with OGI Andy Becher for a routine inspection. Weather: hot and humid following storms the previous day. Chief’s Kevin Hadlock was onsite during our inspection. All six wells were in production with 13-3/8” x 9-5/8”, and 9-5/8” x 5-1/2” annuli plumbed into poly tanks. Annular spaces were monitored for methane using an Altair 5X .3H: 13-3/8 x 9-5/8: 7% LEL, 9-5/8 x 5-1/2: 3% methane.
2514773 Primary Facility 2016-08-24 Routine/Complete Inspection No Violations Noted Arrived @ 11:38. Weather: sunny and pleasant. The purpose of this inspection was to document conditions at the Dacheux 3H, following up on a report of a possible communication on 08/23/16 between this well and the G&S Big Rigger 3H, which was currently being stimulated. Met with Jon Hafer and Gary Bahrke (Chief). At the time of my inspection, the Dacheux 3H was flowing on an open choke at the GPU (64 choke). There was no tubing in the well and the pressure at the wellhead was 1200 psi, which was comparable to the other wells on location. Earlier, flow rate and pressure had dropped. Mr. Bahrke investigated but found no mechanical issues. He dropped soap into the well and it returned 23 bbls of fluid. This was unusual as the well was typically dry.

Dacheux 3H: 13-3/8” x 9-5/8”: 0% /volume combustible gas, 9-5/8” x 5-1/2”: 4%/volume combustible gas There were no violations at this time. Left location @ 12:30.

2538809 Primary Facility 2016-11-23 Site Restoration No Violations Noted Restoration inspection conducted in response to well site restoration reports filed by Chief Oil & Gas, LLC on November 22, 2016.

Interim restoration has been implemented on the well pad. A significant portion of the pad surface has been decompacted and vegetated. The most recent waste report for the subject facilities was submitted to the Department's Oil and Gas Reporting Electronic (OGRE) system on August 8, 2016. A Notice of Termination (NOT) for the erosion and sediment control permit associated with the site was approved by the Department on October 19, 2016.

Based on the information above and site conditions observed during the NOT inspection, these restoration reports are acknowledged.

2577017 Primary Facility 2017-03-30 Routine/Complete Inspection No Violations Noted Six Wells in production status. Six separators within containment. No leaks detected from production equipment. Site stabilized in all areas not needed for access to production equipment or wellheads. A well line was recently run to the well site. No violations noted.
2656903 Primary Facility 2017-11-03 Routine/Complete Inspection No Violations Noted Arrived at 13:00. Weather: cloudy with some rain. No personnel on site. 6 producing wells on location. Annuli venting to 550-gallon polytanks with vent stacks. The stacks were too high to monitor for combustible gas. All cellars filled with gravel. Analog gauges 0-5000 psi.

2680186 Primary Facility 2018-01-09 Routine/Complete Inspection No Violations Noted Arrived at 1:10. Weather: sunny with blue skies. There are six producing wells on this pad. The Pad has been partly grassed. Top gauges on each of the six Wells were approximately 1000 psi (0 to 5000 psi analog). Annular vents were open to poly tanks. I was unable to monitor methane at the polytank openings. No violations were observed during my inspection.
2722871 Primary Facility 2018-04-25 Routine/Complete Inspection No Violations Noted The Dehydration units are being replaced. The larger units are being swapped out for smaller units.
2827061 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 09:46; the weather was ~15°F and calm. The gate was open when I arrived.

There were 6 properly tagged Marcellus wells staged for production. The well cellars were filled with gravel. There were 6 GPUs and 6 produced water tanks onsite and in secondary containment. The well annuli were plumbed to 4 poly tanks not on additional secondary containment.

Areas around the well site were greater than 70% uniform, perennial, vegetative cover. The pad had been reduced and was almost completely revegetated. Some steeper slopes were stabilized with rock lining.

No violations were noted at the time of inspection.
