Frank Ferrana (J.P. Reilly construction contractors) working on site. Location does not have any significant drainage prblems, subsurface is shaley. Perimeter E&S controls are in place, ditches in place as per the plan, sediment retentionl basin in place. Temporary stabilziation good. There were some very minor earth slips along the base of the fill slope, these will be addressed during compaction.

2434086 Primary Facility 2015-12-16 Routine/Complete Inspection No Violations Noted Arrived at 10:34, weather cold and cloudy. There were 3 wells on this location, 1 in production and the other two TD’ed.
2481012 Primary Facility 2016-05-04 Routine/Complete Inspection No Violations Noted The site currently has >70% coverage of perennial vegetation and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

The electrical conductance of puddles was not screened due to the absence of puddles.

The Erosion and Sediment Control General Permit for this site expires on 01/01/2018, and no current well permits exist. Chief has indicated that it plans to partially restore the well pad by the autumn of 2017. In order to remain in compliance with the Department's regulations regarding site restoration (Title 58 Pa. C.S. § 3216), Chief should obtain a new drilling permit or request a 2-year extension of time for site restoration.

No violations were cited during this inspection.

2499045 Primary Facility 2016-06-30 Routine/Complete Inspection Pending 13-3/8" x 9-5/8": 5%, need to get annuli isolated and test again.
2523037 Primary Facility 2016-08-04 Follow-up Inspection No Violations Noted Arrived at 09:00. Weather: cloudy, breezy, cooler. There were no company representatives on site at the time of this inspection. There were 3 wells on this location, 1 in production and the other two TD’ed. This follow-up inspection was prompted by Inactive Well Status applications for the I Harvey 1H and I Harvey 2H. Previously on 6/30/16, (IR 2499045) combustible gas was detected on the I Harvey Unit 2H at the 13-3/8” x 9-5/8” annulus. Typically, the 1” , 13-3/8” x 9-5/8” annular vent T’s into the 2” 13-3/8” x 9-5/8” annular vent, allowing for cross-contamination. To eliminate that possibility in this follow-up inspection, the annuli were open and isolated from each other prior to my arrival on site. Monitoring conducted with Altair 5X gas meter (calibrated: 7/26/16).

2554458 Primary Facility 2017-01-23 Routine/Complete Inspection Pending Routine inspection to assess overall site conditions post-fracking operations. No Chief representatives were present.

The surface of the well pad and access road were extremely muddy due to recent rains, snowmelt, and heavy truck traffic. Recently installed compost filter sock was in place in the field on the downslope side of the access road. No filter medium was present along the upslope sock/ground surface interface as prescribed by the Erosion and Sediment Pollution Control Program Manual (Technical Guidance No. 363-2134-008, Detail 4-1, page 65)(E&S manual). The absence of this filter media allowed sediment to flow underneath the sock in several locations in the vicinity of the guard post. The sediment that flowed under the filter sock continued downslope a short distance and possibly outside of the limit-of-disturbance for the site, but did not affect any aquatic resources or result in a significant accumulation of sediment that would negatively impact land use at that location. 2650768 Primary Facility 2017-10-23 Routine/Complete Inspection No Violations Noted Arrived at 14:08. Weather: overcast, windy. 3 producing wells onsite. Annuli open and venting to 550-gallon polytank. No combustibe gas detected at polytanks. 3 GPUs, 3 produced water tanks. 1 Harvey 3H adding foam. No violations observed.
2784687 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection. The pad size has been reduced and vegetation is established on restored area.

There are 3 properly tagged Marcellus wells staged for production. The well cellars contained water but not bubbling was observed.

There are also 3 GPUs and 3 produced water tanks onsite and in secondary containment. There was very little water in secondary containment. A small amount in the GPU containment had a conductivity of 150.8 µS/cm.

Overall the site is >70% vegetation and no E&S issues were observed. The E&S inspection log and other pertinent paperwork was present onsite. The department recommends inspecting and repairing equipment and storage as needed. At the time of inspection there was some minor rusting on the produced water tanks (Photo #5).

No violations noted.
