Vegetative cover on site is greater than 70% total coverage of perennial plant species. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed. The well pad surface has been decompacted and revegetated as an interim restoration measure. The restoration schedule submitted by Chief indicates that Well Site Restoration Reports (WSRRs) for this site would be filed in April 2016. Title 59 Pa. C.S. § 3216 (2012 Oil and gas Act), which relates to site restoration, requires that a well site be reclaimed within 9 months of completing the drilling of the last well. In order to remain in compliance with the Department's regulations regarding site restoration, a new well permit should be obtained, a request for an extension of time for site restoration should be submitted, or the WSRRs should be filed. No violations were cited during this inspection. |

The site was determined to meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed. The surface of the well pad has been de-compacted and vegetated. All temporary erosion and sedimentation BMPs were removed. Chief also submitted, on December 13, 2016, a well site restoration report for the 1H well ( API No. 113-20108). The most recent waste report for the subject facilities was submitted to the Department's Oil and Gas Reporting Electronic (OGRE) system on August 8, 2016. Based on the information observed during this inspection, the NOT is recommended for approval and well site restoration reports are acknowledged. No violations were cited during this inspection. |

The pad has been reduced in size and vegetation was well established (>70% cover) at the time of inspection. All PCSM BMPs have been installed and appeared to be functioning as designed. There are 3 properly tagged Marcellus wells staged for production. There are also 3 produced water tanks, 3 GPUs, and 1 compressor onsite and in secondary containment. There was some minimal corrosion noted on the produced water tanks (Photo #12). There was also corrosion noted around a weld on pipeline leading to the GPUs (Photo #9). Near the wells there were 2 containers with hoses that were not labeled (Photo #6). A tank of used motor oil was near the compressor with a very faded hazard label(Photo #11).
