Upon arrival, drill rig completed the installation of conductor and surface casing and was removed from site. The log book was located in the entrance way mail box with the frac transportation hour¿s invoice. On the cover of the log, Ziegenfuss noted 3/23/10 start date. DEP confirmed start date with drilling company (Ziegenfuss Drilling (908-788-5100). Chief failed to notify DEP prior to 24 hr notification. E/S plan and sign at the entrance of the access road. Sign was missing permit number. E/S plan was missing site plot and E&S controls plan. Additional site activity was occurring around pad with no E/S plans and limited E/S controls. On the site pad, two locations of oil staining/oil loss to the ground from location of where the drill rigs would be located. No attempt was made to clean the oil release
1877308 Primary Facility 2010-04-08 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of lined freeboard. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. All erosion control BMP¿s appeared to have been implemented properly. Noted that earthmoving activity was ongoing adjacent to the drill pad. Spoke with operator Matt Barch via phone regarding this matter and he stated that this was work to construct the frac pit and that an ESCGP-1 covering this activity was obtained and was with the contractor performing site construction at that time.

Pit aligned with 20 mil plastic aligned drilling pit and frac pit aligned also and security fence. No free board issues with pit. Well connected production equipment. Swab rig was present at the wellhead. Heavy equipment was on-site to start restoring site and pits.
1884554 Primary Facility 2010-05-12 Routine/Complete Inspection Violation(s) Noted At the time of inspection, the well was drilled not yet in production. Site was properly signed with E&S plan. Violations were noted. Two foot pit freeboard was exceeded on pit and IW was released onto the ground outside of pit. Noted various bmp¿s were installed filter sox¿s, diversion ditches, sediment traps, as well as an infiltration berm, which appeared to be functioning properly. Although it is recommend that additional rock or other stabilizer be added to diversion ditches leading to sediment trap(s) to prevent soil erosion into channel. Access road appeared stable and free of E&S issues.
1918459 Primary Facility 2010-10-06 Complaint Inspection No Violations Noted At the time of inspection, no E&S or IW discharge issues at the site. Pit aligned with proper plastic and no freeboard issues. Keystone Vacuum was removing fluids from pit. E&S controls were in place. Chief was in the process of flow back and flaring gas well from Fracking procedure. Due to Chief policy of ¿fire retardation suites¿, DEP was not allowed to enter the site pad but was able to view the flare and site conditions from the Security station. Based on visual view of the site, the flame was showing no smoke or air quality problems. The site showed no signs of stress vegetation or air quality problems. DEP called Chief (JD Slater ¿ Compliance Manager). He reviewed the log for Sunday and showed no reports of site air quality concerns.

Pit contents and liner have been removed offsite, operator agreed to supply department with manifests for all materials removed from pit (including liner)--operator unable to account for liquid removal prior to encapsulation as it was handled by previous owner. Operator also has plans to file an Amended Restoration Report to indicate that the previously encapsulated pit has been removed off site. Based on current observation, dept concurred w/ opr proceeding w/ plans to backfill pit, it visibly appears contimanted soils/materials removed
2078166 Primary Facility 2012-06-20 Routine/Complete Inspection No Violations Noted Inspection result of operators notification that previously encapsulated pit has been successfully removed and backfilled. During today's inspection, department noted that area of previously encapsulated pit had been back filled with clean fill material, but due to recent heavy rains it appears that soil has "settled." Due soft nature of the fill it was recommended that operator add additional fill prior to rocking area to ensure that material is properly compacted and stabilized as operator has plans to drill another well on location and drill rig may be placed over area recently back filled. Again, department is currently awaiting final response from operator including disposal records, amended restoration report, as well as written report on activities undertaken for removal of previously encapsulated drill pit.

2136131 Primary Facility 2013-01-31 Incident- Response to Accident or Event In Compliance with Policy Admin Ispt. Spoke to Chevron representative Curtis Small via phone on 1/31/13. According to Mr. Small, on 1/30/13 while HWR company was loading excavator parked where access road branches off towards impoundment, hydraulic line burst, spilling approximately two quarts of hydraulic oil onto access road. Spill was immediately soaked up and the impacted gravel shoveled into drum. Samples taken, will dispose of in landfill. It appears situation was handled according to policy and a follow-up inspection will be conducted in future to ensure full compliance.

Security on-site.At time of inspection, in process of skidding rig to resume drilling. 1H had been drilled. 2H, 3H, 4H, 5H, 6H all spudded, TDs not yet reached.Observed small staining on gravel on front edge of rig containment when standing at DSM trailer and noted large snow/gravel pile against brine tanks in back corner of pad which could lead to a potential pollution event by falling into ditches after thawing.Spoke to DSM Ken Silvis who agreed to place the affected gravel on containment and agreed to remove the snow pile.Noted erosion issue from previous inspection on 11/28/12 (ID 2121603) was addressed by utilizing an erosion blanket for stabilization and removing sediment from ditch.Rig and on-site tanks on containment. Tanks and containment appear structurally sound. Access road adequately rocked and no accelerated E&S issues were noted in relation to well pad or access road. Department recommends that Operator remove debris from rock-lined ditches.
