2033013 Primary Facility 2012-01-09 Routine/Complete Inspection No Violations Noted An inspection was conducted on 1/9/12 at the DCNR Tract 001 - Pad H site in Sweden Township, Potter County. The site ID sign, ESCGP, and perimeter of the site were observed. The ground was frozen and mostly snow covered. No evidence of accelerated erosion was observed. Where visible, the slopes appeared stable and vegetated. In several areas on the pad surface, there were small amounts of a balck, shiny rock-like material spilled. Doug Kepler was contacted and provided pictures of the spilled material. Mr. Kepler informed the Department that the material is used for sandblasting and had been cleaned up the morning of 1/11/12. Recent weekly inspection records for this site were not in the mailbox for this site, this issue needs to be addressed. No new violations were cited during this inspection. - MRN
2164796 Primary Facility 2013-05-01 Routine/Partial Inspection No Violations Noted A routine inspection of the DCNR 001 Pad in Sweden Township, Potter County was conducted on May 1, 2012. All E&S controls remain in place and a record of inspections on behalf of Seneca was located in the mailbox with E&S plan. The last inspection documented on behalf of Seneca was listed as 4/23/2013. The site was stabilized and no evidence of accelerated erosion was observed. Several puddles on the pad surface were screened for elevated conductance; no elevated reading were observed. The 2H and 7H wells have been drilled and the 1H, 3H,4H,5H,6H,8H,9H,10H,11H,12H,13H,14H and 15H will are listed as Permitted But Not Materialized. No violations were cited during this inspection.

2409571 Primary Facility 2015-09-22 Routine/Complete Inspection No Violations Noted The Department inspected the DCNR 001 Pad H on 9/21/15 @ 1300 hours. The weather was sunny 75 degrees. A site ID sign was observed at the entrance to the well site. The E&S plan and PPC plan were located in the mailbox. The site was last inspected by company personnel on 8/25/15 @ 700 hours. The site status was listed as “in production”.

2582405 Primary Facility 2017-04-10 Routine/Complete Inspection No Violations Noted On 4/10/17 at 1402 hrs I conducted a routine inspection at the DCNR 001 Pad H well site. It was mostly sunny and 73 degrees F. No one was present during my inspection.

Two wells, the 2H and 7H, have been drilled and are in production; drilling of the wells was completed in November 2010. There have been no new well permits issued for the site for several years. It does not appear that the well site has been restored and the pad remains at nearly full size (approximately 300 feet x 600 feet), with the exception of the southeast pad corner where the impoundment was formerly located. Interim restoration of the well site should be completed as per 25 Pa Code Section 78a.65(a)(1).

On 2/29/16, Seneca submitted a Well Site Assessment Summary to the Department. The Summary milestones for Pad H included submittal of Well Site Restoration Reports (WSRRs) and the Notice of Termination (NOT) for the ESCGP. To date, neither the NOT or WSRRs have been received by the Department. If Seneca does not intend to conduct int

The wellhead assembly all fittings and flow lines were tested for gas. The results of this testing was no gas was detected on the wellhead assembly or the well area.
2725211 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted A routine inspection at the DCNR 001 Pad H well site and the 1V 50258 monitoring pad was conducted on May 1, 2018. It was sunny and 68 degrees F. No one was present during this inspection.

Two wells, the 2H and 7H, have been drilled on Pad H and appear to be capable of producing. Drilling of the wells was completed in November 2010. The last active well permits expired in 2012. It does not appear that Pad H has been restored and the site remains at nearly full size (approximately 300 feet x 600 feet), with the exception of the southeast pad corner where the impoundment was formerly located. 2767681 Primary Facility 2018-08-27 Routine/Complete Inspection No Violations Noted At the time of this inspection this well appeared to be in production. A digital gauge on the casing read 1127 psi( pounds per square inch )and a digital gauge on the tubing read 785 psi. A mechanical gauge on the 3" production line leaving the well head assembly,going into the ground and then into the production equipment read 900 psi. No violations noted at this inspection.
