Once the site is determined to meet the requirements of permanent stabilization, as per 25 PA Code § 102.22(a), a Well Restoration Report, as required under 25 PA Code § 78.65 (3), should be submitted to the Department. No violation were cited during this inspection.

The wellhead assembly all fittings and flow lines were tested for gas. Results of this testing were that gas was detected on the wellhead assembly where the fitting with inside threads is threaded onto a casing approx. 5" in diameter out of the ground. No other gas was detected on the wellhead assembly fittings or the well area.
2760685 Primary Facility 2018-08-01 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was not in production as visually observed and noted. All valves associated with this wellhead assembly and well area were shut off. Well area was overgrown with excess vegetation. This well is approximately a 12" casing out of the ground reduced to approximately a 7 1/2" casing with a 7"x2"x2" tee style fitting with a hammer style fitting on top to tighten it to the approximate 7 1/2" casing out of the approximate 12" casing. The tee style fitting is threaded into the casing and the hammer style fitting is used to tighten the wellhead assembly fitting to the casing. A 2" valve that is shut is on one side of the tee fitting and a production line also 2" with a 2" valve that is shut is on the other side of the tee fitting. The photo shows bubbling out of the top of the hammer fitting where the positive combustable gas reading was noted. The bubbling was visual but not audible. Bubbling was too numerous to count. This appears to be leaking as a result of a faulty valve
2855759 Primary Facility 2019-03-19 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was producing. The wellhead assembly is a casing out of the ground approximately 8" in diameter with a smaller diameter casing approximately 7" inside of it. The smaller diameter casing has a tee (t) style fitting approximately 5" x 2" x 2" threaded on to it. One side of the tee style fitting is an approximately 2" diameter valve that is shut. On the other side of the tee style fitting is an approximately 2" diameter valve that is open and connects to what appears to be a production/flow line. The line goes away from the wellhead assembly and into the ground. The top of the tee style wellhead fitting is a hammer (how it is attached) style fitting with approximately 2" diameter vale that connects to the wellhead assembly. No violations noted at this inspection.
