2630634 Primary Facility 2017-08-29 Site Restoration No Violations Noted On 8/29/17 at 1336 hrs I conducted a restoration inspection of the SGL 59 HHF5 well site. No one was present at the site. It was partly sunny and 67 degrees F.

The HHF5 well was drilled by Allegany Enterprises. The well was tagged and the area around the wellhead mowed. A faint hydrocarbon odor was noted near the wellhead. The site is restored and permanently stabilized as per 25 Pa Code Section 102.22(a). The Well Site Restoration Report (WSRR) was received on 5/4/17; it was reviewed and acknowledged on 8/29/17. According to the WSRR, an encapsulated drill cuttings pit is located 20 feet west of the wellhead.

Ownership of the well was transferred from Allegany Enterprises to Roulette Oil and Gas on 5/31/17.

No violations were cited during the inspection. I departed the location at 1345 hrs. TLT

2670760 Primary Facility 2017-12-13 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. All wellhead assembly fittings and flow lines were tested with a gas meter with the results of the testing being no gas detected on the wellhead assembly or the well area.
2710393 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. The wellhead assembly, all fittings , flow lines and vents were tested for combustable gas. Results of this testing was that no combustable gas was detected.

2855753 Primary Facility 2019-03-19 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was producing. The wellhead assembly is a casing out of the ground approximately 8" in diameter with a smaller diameter casing approximately 7" inside of it. The smaller diameter casing has a tee (t) style fitting approximately 5" x 2" x 2" threaded on to it. One side of the tee style fitting is an approximately 2" diameter valve that is shut. On the other side of the tee style fitting is an approximately 2" diameter valve that is open and connects to what appears to be a production/flow line. The line goes away from the wellhead assembly and into the ground. The top of the tee style wellhead fitting is a hammer (how it is attached) style fitting with approximately 2" diameter vale that connects the wellhead assembly to the pump jack. No violations noted at this inspection.
