2821737 Primary Facility 2019-01-02 Routine/Complete Inspection No Violations Noted This well is an approximate 5"casing out of the ground with an approximate 4" casing inside of the approximate 5" casing. There is an approximately 4" x 2" x 2" tee (T) style fitting threaded on top of the approximate 4" casing. Both of the approximate 2" openings on the sides of the tee (T) style fitting are open. All production and or flow lines are disconnected and no longer attached to the well or wellhead assembly. On top of the well is an approximately 3' high x approximately 20" in diameter steel sleeve that contains a piston that was used to pump down the production of the well with air pressure supplied by a air compressor. This well is no longer in production and will be plugged at a yet to be determined date. No violations noted at this inspection.
