The access road and location are stoned, and culverts are in place as per the erosion and sedimentation plan. There is a pit on location; the pit has some water in it. The well permit is visible in a tree at the entrance to the site.

Lined pit in use and has water and drill cutting. Permit posted and paper work are in doghouse. No E&S problems evident at this time.

There is a 20 mil lined pit on location containing water, oil and frac fluids. No spills or leaks are noted at this time.
2676564 Primary Facility 2018-01-02 Complaint Inspection Violation(s) Noted The Operator called January 1 to inform of an approximately 3 barrel spill near the PHR Wt 3131 42 well. The production line split due to a frozen line further up the line, forcing oil up through the frozen soil to the surface. The operator has exposed the line at this point and removed contaminated soil to stockpile on the site. They used a steamer on 1/1 to vacuum up as much oil as possible. Several barrels of product and absorbent materials are on site. More soil is set to be removed so that the line can be repaired to allow the wells to be put back into production.

They have hired a contractor (EADS Group) to take samples to determine whether the spill has been adequately cleaned. They took two samples around 1030 and are awaiting results.
