2764646 Primary Facility 2018-08-15 Complaint Inspection Violation(s) Noted The operator called to report an unknown quantity of oil released from a line near the Wt 3131 #13 well on 8/15/18. It was determined that a line under pressure developed two small holes, which allowed oil to leach into the surrounding soil for an unknown length of time.

Oil then traveled underground approximately 150 feet before emerging in an UNT of South Branch Kinzua Creek. It followed this tributary downstream for about 1000 feet before dissipating in the natural flows.

The operator has exposed and patched the line temporarily. They also had a crew of 6-7 employees working in the stream to build siphon dams, add booms and absorbent pads and collect product as possible. The operator plans to continue to work in the stream as long as needed to remediate this spill.

2766233 Primary Facility 2018-08-22 Follow-up Inspection Outstanding Violations - Viols Req'd This was an inspection performed with PFBC officer Thomas McMann to check on the progress of cleaning up the stream channel. The operator had a crew of three employees cleaning loose oil and boom and siphon sites, as well as changing out pads and booms. The stream channel looked to clear significantly following the flush of water from recent heavy rains. Pockets of oil are still visible in backwater areas and sheen is still visible on the surface and under rocks in the channel.

Continued monitoring of this spill cleanup will be done as necessary.

2770312 Primary Facility 2018-09-04 Follow-up Inspection Outstanding Violations - Viols Req'd This is a follow up inspection to an 8/15 spill of crude oil into the ground and an UNT of South Branch Kinzua Creek from the Wt 3131 13 well.

The operator has continued to clean oil from the stream using skimmers at boom and siphon sites. It appears that less product is entering the stream although a significant sheen can still be found when agitating the channel. Continued cleanup will be done and monitored by the Department as possible.

2804034 Primary Facility 2018-11-16 Follow-up Inspection Outstanding Violations - Viols Req'd This was an inspection to monitor cleanup of a spill into UNT South Branch Kinzua Creek that occurred 8/15/18.

Some oil is still collecting on booms and siphon dams. However, leaves and sediment have clogged most outlet pipes of siphon dams, which is causing water to overtop these controls. In addition, high flows have allowed booms to float to riffle areas, which allow oil to bypass collection. A heavy sheen was still noted in areas of leaf collection in particular.

Continued monitoring will be done as possible, as winter weather has arrived and heavy snow has fallen on this site in recent days.
