At the time of the inspection, equipment was being assembled to (toe) fracture the Wissler South Unit 1H (081-21460) The spill was located at the back (south/southwest side) of the pad behind the working tanks (off containment). An area approximately 4'X10' had snow and slushy ice that was slightly brownish/off color. Field conductivity was approximately 1480 umhos/cm over the majority of the visibly effected area and approximately 8,140 umhos/cm at the highest. After returning to the company trailer we asked the company representatives to vacuum the spill area since solid ice was visible under the slushy snow and ice. The company representatives explained that they had been vacuuming the area for some time and were planning to continue until the frac-job is over. Violations Noted to 081-21460.

I entered the site at 12:40. At the time of the inspection, Wissler South Unit 1H (081-21460) was in the flowback phase of operations. Oil States (subcontractor) was onsite monitoring the temporary production equipment and sand separators. Other contractors were onsite removing equipment. Containment was cut back from the full Frac Job coverage to a smaller containment around the well heads, temporary production equipment, and sand separators. Frac tanks (containing flowback) were lining the northwest and southwest sides of the well pad. water in puddles on the pad surface was between 495-1110 umhos/cm while water in containment was between 345-450 umhos/cm. The entire pad and access road surfaces were covered in 6-8 inches of mud. Mud was flowing off the access road and into the basin near the entrance to the site. PCSM has been implemented and the 2-year extension (delaying restoration) was approved on 2/24/2016. I recommend stabilizing the site and maintaining

At the time of the inspection, one well was in production. The annular vent is plumbed to a poly tank. The production tank is in containment and is in good condition. PCSM was previously implemented and a 2-year extension was approved on 2/24/2016. The 2-year extension expires on 2/24/2018. I recommend evaluating the well site restoration plans. No violations noted.

Annular vents open and plumbed to 550-gallon poly tank. No methane detected at tank mouth. No violations were observed during this inspection.
2758853 Primary Facility 2018-08-01 Routine/Complete Inspection No Violations Noted Weather was 73 degrees and cloudy at time of inspection. Met with Chief representative Floyd Roberts on location. Well was shut in at time of inspection due to completion work being performed on the other 2 wells on the pad. Well had a 0-5k psi pressure gauge attached above the swab valve that read approximately 1200 psi at time of inspection. There were 2 annulus vents plumbed to surface that were both closed in at time of inspection. 0% combustible gas was detected in any area around the well at time of inspection. No violations noted at time of inspection.
2784233 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted Site was in flowback phase when I arrived; 2 temporary separators and heaters were hooked up to the wells. The site had a large number of puddles, both on and off containment. These all showed low conductance (.18-.33 mS/ cm). Water had also built up under the liner, creating small spots where the liner was pushed up by the water. Berms had been removed from the sitewide containment, but no equipment remained on that liner. The containments for the Baker tank batteries on site appeared to be in good condition; however, on the outside of the berms for these containments, the liner was stretched such that it was not making contact with the ground for 1-3', increasing the possibility of a tear. The site has an existing pipeline, and the wells will be put directly to sales after flowback is complete.
