2469545 Primary Facility 2016-03-25 Routine/Complete Inspection No Violations Noted At the time of the inspection, the Myers Unit 1H (081-20279) and 7H (081-20790 were in production. The well pad area was relatively well vegetated but some erosion of gravel off the well pad surface (photo attached) was noted. The well site has not been restored and the Well Site Compliance Information Request reply (submitted 9/21/2015) says the well site was permitted under an ESCGP-1 and a new drilling permit will be submitted in the fall of 2015. The Department has no record of an ESCGP issued for the site and no well permits have been issued for the site since 12/8/2011. The Department’s records indicate five active wells exist on the site, however, only the two wells mentioned above were located on the pad. The well pad remains at its original size. I recommend you apply for an ESCGP-2 to cover this since it has not been restored and was part of a larger disturbance. Please also provide an update on the status of the three active wells that were not found onsite.

2505447 Primary Facility 2016-07-27 Routine/Complete Inspection No Violations Noted Arrived @ 15:00. Weather: hot. Met with Bird Smith (EXCO), also onsite was a Guzzler vac truck (Fast Rig Support), and Greg Miller (Catawater). At the time of my arrival workers had already sucked open the cellar of the Myers 2H, revealing two annular pipes, and a vent for the 9-5/8”. They were determining what piping would be needed to vent the annuli at the surface. While I was onsite, the cellar of the 3H was partially sucked out. The arrangement of the annular pipes was similar. The plan was to finish the work on 7/28/16 and leave the annular vents open.

There were also two completed wells on location the Myers 1H, which was shut-in, and Myers 7 H which was in production. No violations were apparent at the time of this inspection. Left location @ 16:10.

2509273 Primary Facility 2016-07-29 Follow-up Inspection Pending Arrived @ 08:30. Weather: hot and humid. Met briefly with John Orwig, lease operator, who was finishing up as I arrived. At the time of my inspection, there were 2 wells in production and 3 top-holed wells. Gravel that filled the cellars of these top-holed wells was being removed during my previous inspection of 07/27/16. The purpose of this inspection was to evaluate the annular vents that were uncovered after the cellars were vacc’ed out. An Altair 5X gas meter (calibrated on July 26) was used to monitor annular vents were monitored for combustible gas.

Myers 6H: Top-holed well with 2 vents. At the time of my inspection the identity of these vents was unclear. Vent on the east side of the well: no combustible gas detected Vent on the west side of the well: 100% combustible gas, minor but detectable flow. The result of this inspection is, “PENDING,” due to uncertainties concerning the identity of the annular vents. Left @ 09:40.

2518168 Primary Facility 2016-08-08 Follow-up Inspection Violation(s) Noted Arrived @ 10:30. Weather: hot. Met with Bird Smith, to follow up on my 7/29/16 inspection (IR2509273) which found combustible gas on an annular vent whose identity was uncertain at the time of the inspection. For the three top-holed wells, he identified the annular vent on the east side of the well as the 20” x 13-3/8”. The vent on the west side was identified as the 13-3/8” x 9-5/8” annulus. These vents were monitored for combustible gas using an Altair 5X gas meter (calibrated on 8/8/16). 20” x 13-3/8”: no combustible gas detected

13-3/8” x 9-5/8”: 100 combustible gas /volume, minor, detectable flow.

2534244 Primary Facility 2016-11-02 Follow-up Inspection Outstanding Violations - No Viols Req'd Arrived @ 8:45. Weather: foggy, warm. There were no personnel on site at the time of my inspection. There was a total of 5 wells on site: three top-holed wells: Myers Unit 2H, 3H, and 6H, and 2 wells capable of production, Myers Unit 1H & 7H. The producing wells were off-line at the time of my inspection. Annular vents on the 5 wells were monitored for combustible gas using an Altair gas meter (calibrated 11/2/16).

Myers Unit 6H: 20” x 13-3/8”: 0% combustible gas/volume; 13-3/8” x 9-5/8”: 100% combustible gas/volume. As I was leaving the site the lease operator was arriving to bring the producing wells back online. Left location @ 9:15. .

Myers Unit 3H: no combustible gas detected on either annulus. Myers Unit 6H: 20” x 13-3/8”: 0% combustible gas/volume; 13-3/8” x 9-5/8”: 50% combustible gas/volume. The result of this inspection is, OUTST, because a violation for defective cement on the intermediate casing string was cited on 8/8/16 (IR # 2518168).

Annular vents on the Myers 6H and 2H were monitored for combustible gas using an Altair gas meter (calibrated 1/26/18).

Myers Unit 2H: 20¿ x 13-3/8¿: 0% combustible gas/volume; 13-3/8¿ x 9-5/8¿: 22% combustible gas/volume. Myers Unit 6H: 20¿ x 13-3/8¿: 0% combustible gas/volume; 13-3/8¿ x 9-5/8¿: 25% combustible gas/volume. Meyers 1H, in production. Discoloration and rust appear to be associated with a minor leak of produced water.

For the 6H, the result of this inspection is, OUTST, because a violation for defective cement on the intermediate casing string was cited on 8/8/16 (IR # 2518168).
