There was no activity on the site. There was one well in production on the pad. The well was tagged and there was a grate over the cellar which had frozen water, no BAW. There are steel plates covering the outer annuli with ½” stainless steel vent lines at the surface. The lines are labeled.

Methane gas was monitored with an Altair 5X gas meter calibrated on 1/4/2016. 20” x 13-3/8” – 0% methane 13-3/8” x 9-5/8” – 0% methane 9-5/8” x 5-1/2” – 100% methane. The 9-5/8” x 5-1/2” annulus is plumbed to a poly tank. Methane was also detected at the tank vent. A previous inspection (IR# 2095705), cited a violation for defective cement on the 9-5/8” x 5-1/2” annulus because there was evidence in the well record to suggest that the production casing was cemented to surface. No additional violations were noted during this inspection. Consequently, the result of this inspection is: OUTNO.

There was one well in production on the pad. It was surrounded by concrete barriers. The well was tagged and there was a grate over the cellar.
