Your reply to the Well Site Restoration Compliance Information Request indicates you will apply for a well permit in 2015 but, no well permits have been issued for the site since 11/30/2010. The well site was re-permitted with an ESCGP-2 in November 2015 to include stormwater management but those revisions have not yet been implemented. Currently, erosion is noted throughout the cut slope of the well pad and rock-lined ditches. The sedimentation basin’s slopes have little vegetation and several inches of sediment have accumulated in the bottom. The well site has not been restored. I recommend you implement the ESCGP-2 measures and address the erosion and lack of vegetative cover on the cut slope of the well pad.

2502499 Primary Facility 2016-07-18 Routine/Complete Inspection No Violations Noted Routine Inspection; 7/19/2016. At the time of the inspection, the Arthur Unit 1H (081-20353), Arthur Unit 2H (081-20354), and Arthur Unit 3H (081-20452) were in production. The PCSM (Post Construction Stormwater Management) plans were recently implemented as part of the ESCGP-2 issued in November of 2015. Construction has been completed and the area was seeded and stabilized. Stabilization was somewhat sparse in some areas. I recommend keeping an eye on these places and stabilizing further as needed. No Violations Noted.
2544956 Primary Facility 2016-12-07 Routine/Complete Inspection No Violations Noted Arrived @ 15:00. Weather sunny. No company representatives were present at the time of the inspection. A total of 3 wells onsite, all in production. All three had open annular vents for the 13-3/8” x 9-5/8” , and 9-5/8” x 5-1/2” annuli. The vents were monitored with an Altair 5X gas meter (calibrated on 12/6/16). No combustible gas was detected in any of the annular vents.
