2442318 Primary Facility 2016-01-20 Routine/Complete Inspection No Violations Noted Remarks: Arrived @ 11:00. Temperatures in the 20’s, calm and cloudy with a light coating of snow. There were 6 wells on this location in a linear array surrounded by concrete barriers.

Red Run Mountain 736 2H. The well was capped. There was a 2” pipe emerging out of the cap with a levered ball valve and a needle valve. The annulus between the intermediate and surface casing was filled with ice. The well tag was illegible because it was frozen into the annular ice. The identity of the well was determined by a process of elimination. A review of Department records indicated that this well was top-holed, Swepi acquired it, the well permit expired 3/2015, and there were no well records filed for the well. Jason Shoemaker (SWEPI) indicated that Swepi would, within weeks, be applying for a new permit, and would submit well records. The Department recommends replacing the well tag to reflect the well’s ownership. There were no violations at this time. Left location @ 11:50.

2447432 Primary Facility 2016-01-05 Compliance Evaluation No Violations Noted I arrived onsite at 11:09 to evaluate the discharge that was documented in a NOV (Notice of Violation) issued 9/30/2010 to East Resources Management. Samples 3440262, 3440263, and 3440264 collected on 1/5/2016 demonstrate the water leaving the well site and entering Red Run immediately downstream of both the well pad and pipeline meets the 25 PA Code § 93 water quality criteria standards. A two year extension on the restoration of the well site was approved on 4/29/2015 and will create an infiltration basin where the two sedimentation basins are currently located. Six wells on the pad have been drilled (one is conductor only) and never produced even though pipeline is connected to the well pad. The concern for the wells onsite has been referred to the Oil and Gas Inspector for the area. Violations documented in the Department’s letter to East Resources Management on 9/30/2010 are considered closed.

I entered the site at 11:46 from the access road off of the existing hunting club roads. Six wells onsite have never produced although pipeline is constructed to the well pad. The wells are all tagged properly. PCSM has been permitted but has not yet been implemented. The site is well vegetated and no E&S issues were noted. No Violations Noted.

2669614 Primary Facility 2017-12-11 Routine/Complete Inspection No Violations Noted Weather was 28° and snowy at time of inspection. There are six wells on location. Well is open to atmosphere by a 2 inch ball valve attached to a nipple on the 9 5/8 inch casing. Wells are listed as active in E-facts, however they do not appear to have ever been completed. 13 3/8 casing collar is below the top of the conductor. No cement is visible at surface in the 13 3/8 x 20 annulus or the 9 5/8 x 13 3/8 annulus at time of inspection. Both annuli are cover with frozen water. No combustible gas was discovered in any location at time of inspection.
2718050 Primary Facility 2018-04-09 Routine/Complete Inspection No Violations Noted Routine complete inspection; 4/9/2018.

The Post Construction Stormwater Management plans have been implemented and previously disturbed soils are stabilized. Perimeter E&S controls have been installed and are functioning properly. Standing water was visible in infiltration basin #1 (at the well pad) and water was still discharging steadily through the under-drain. The wells onsite are not in production. No violations noted.

2760934 Primary Facility 2018-08-02 Routine/Complete Inspection No Violations Noted Weather was 73° and raining at time of inspection. There were no Rockdale Marcellus representatives on location at time of inspection. Well has intermediate casing cemented and nippled down to a 1/2" needle valve. Needle valve was open at time of inspection. 0% combustible gas was detected on the valve or in the area surrounding the well at time of inspection. No violations were noted at time of inspection.
