1810273 Primary Facility 2009-07-06 Compliance Evaluation Outstanding Violations - No Viols Req'd On 5/28/09, I inspected this well pad and noted that some of the soil disturbances were not stabilized, and also discovered that there was no E&S plan available for review. The inspection was entered under # 1801102. Andy Klinger accompanied me during this inspection. All of the soil disturbances are now well mulched and vegetation is starting to grow. However, an E&S plan is still not available for review. Therefore, Seneca Resources Corporation remains in violation of 25 Pa. Code § 102.4(b)(7). I have a call into Doug Kepler to discuss the issue.

One plugged well with a candy cane vent surrounded by concrete barriers. Field monitoring detected 6% to 10% methane using an Altair 5X gas meter calibrated on 04/27/16.

A review of previous inspection reports indicates that there were three efforts to plug this well. These plugging efforts appear to have diminished the presence of combustible gas, but failed to stop it. The result of this inspection is OUTNO, because a violation for defective cement was previously cited on 1/5/2012 (IR# 2034855). Left location @ 14:55.

2527466 Primary Facility 2016-10-13 Plugging(Includes Plugged/Mined Through) No Violations Noted On October 13, 2016 at 10:20 am I was in Lewis Twp. Lycoming County to conduct a routine inspection of (2) plugged gas wells. I discovered a monument for the DCNR 100 38940 plugged well (API Permit# 081-20141) drilled in 2009 to TD @ 7474’ (see well record). The monument was a 2” candy cane vent marked “Date Plugged 8-15-14”. I used an Altair 5x gas meter to check for gas at the vent; not gas. I could not find a monument for the other plugged well API Permit# (081-20030.) PA Gas Mapping shows this well is about 30’ east of the Seneca well, and was drilled and plugged in 1988 by Mat Oil & Gas Exploration. Well records explain the well was drilled to 127’, hole made a lot of water and was caving, 11 ¾” was set at 67’ not cemented, plugged back from TD @127’. The plugging certificate for this well makes reference to a 4’ high 2” candycane vent monument which must have been removed.

One plugged well with a candy cane vent surrounded by concrete barriers. Field monitoring failed to detect methane using an Altair 5X gas meter calibrated on 03/28/17.

A review of Department records indicates that the well was plugged on 08/15/14. Left location @ 14:45.

2641327 Primary Facility 2017-09-27 Site Restoration No Violations Noted The well site has been restored to the approximate original contours established after the plugged well site was originally restored in 1988 after Mat Oil and Gas well (081-20030) was plugged. The site is permanently stabilized and the site has been restored to approximate original contours and accommodates an access road through the site to a cabin to the north. I recommend approving the Well Site Restoration Report. The restoration reporting fulfills the requirements of the well permit unrelated to the requirements of the landowner. No Violations Noted.
