2564382 Primary Facility 2017-02-23 Routine/Complete Inspection No Violations Noted This well was drilled in an abandoned agricultural area off of SR 536 in September 2005. The well is equipped for production and house gas with a brine tank attached and

has been tagged with the correct DEP permit number. A bull plug has been installed in the brine tank drain valve. The site has been graded to final contours and stabilized with vegetation. The brine line from the separator to the brine tank has rusted through. Animal digging at the base of this line indicates that it has been leaking for some time. This line must be replaced. The well valves were closed at the time of inspection, and there was no chart in the meter house. Production reporting for 2015 indicated 1,269.91 Mcf over 162 days. Information regarding 2016 production was due by 2/15/2016, but has not yet been received from the operator. The long access has several areas of accelerated erosion where stone and water bars should be added for stabilization, including two sections of the road that are very steep. These areas must
