2781214 Primary Facility 2018-09-26 Routine/Partial Inspection No Violations Noted At the time of inspection, Plants & Goodwin were onsite trying to get rig to well. Area was extremely muddy so a dozer was pushing equipment around. It was decided to shut down until things dry out before proceeding. I did not attempt to get to well due to this. GPS coordinates have not been checked as of yet.
2781635 Primary Facility 2018-09-28 Routine/Partial Inspection No Violations Noted At the time of inspection, no activity was taking place. The access to the well is extremely tight, so larger vehicles need to go around the blueberry farm to turn around. It was still very muddy there so plan was to wait until Monday to move in. Rock was supposed to be delivered soon for the area. The well is right behind a house and beside a garage. Matting boards were set up in the area. Landowner had some concerns about his water well and cement dust.
2785451 Primary Facility 2018-10-04 Routine/Partial Inspection No Violations Noted At the time of inspection, Plants & Goodwin had rig #4 over the well. They had just got moved in and set up yesterday. During the inspection, they were setting 4 1/2" into the well on a packer. It will be set to between 700-750'. This was to shut off the water. There was a water tank brought in and a large dumpster with a liner.
2788315 Primary Facility 2018-10-11 Routine/Partial Inspection No Violations Noted At the time of inspection, Plants & Goodwin rig was pulling tools out of well. The had just broke through a bridge plug that was 75' thick. They wanted to let the hole settle in hopes that there would not be any caves dropping on the tools. Current depth was 1208'.
2792707 Primary Facility 2018-10-17 Routine/Partial Inspection No Violations Noted At the time of inspection, Plants & Goodwin rig was cleaning out the well. Things have been progressing and they have made it to a depth of about 1800'. They will continue what they've been doing.
2797869 Primary Facility 2018-10-25 Routine/Partial Inspection No Violations Noted At the time of inspection, no activity was taking place. The 6" casing parted when pulled. Was unable to retrieve casing below where it parted so it was perforated this morning. 115 sxs cement were added going from 1225' to 665'. 2320' was attainable bottom due to getting stuck several times. This was plugged properly as well.

The rig was still onsite. The roll off was closed on top. Plans were to be back on Monday to fill with pea gravel and place a marker underground.

2798169 Primary Facility 2018-11-05 Routine/Partial Inspection No Violations Noted At the time of inspection, Plants & Goodwin were onsite doing maintenance on one of their trucks. The rig has been taken to the next location. Tubing and casing were still onsite.

Currently, they were waiting for pea gravel to arrive so they could fill the well to about 10' below ground level. Then, 2 sxs cement will be added before cutting off the conductor pipe and placing a marker underground.

2800897 Primary Facility 2018-12-17 Administrative/File Review No Violations Noted At the time of inspection, a mini-excavator as on-site reclaiming the area. Well has been cut off and buried below ground level. The matting boards have all been taken offsite. Limestone was added to the driveway.
