2207095 Primary Facility 2013-08-01 Administrative/File Review In Compliance with Policy Meeting requested by Wilmoth rep Jason Wells in response to inspection record 2183368 for violations issued for failure to construct drill pit so that bottom of pit is a minimum of 20 inches from the SHGWT. Penalty not yet assessed. Mr. Wells stated that while constructing the pit, the SHGWT was not encroached upon. Based on the Dept's resources and Opr's E&S plan, the SHGWT at this site is shallow and was consequently encroached upon; hence, Department stated that Operator needs to prove and document that the SHGWT is greater than 20 inches from the bottom of the pit. Disposal of drill cuttings as per current regulations were also discussed.In addition to myself and Mr. Wells, also present were Dept reps April Weiland, Frank Calderon, Rickey Hoover, and Mark Millward.

2223651 Primary Facility 2013-11-20 Routine/Complete Inspection No Violations Noted At time of inspection, well equipped for production. Site ID sign at access road. Site has two (2) brine tanks, both appear sound. Pump jack operating during inspection. Site has a grass-lined ditch that runs toward and behind well. Site has been seeded and catch of growth is evident. Site is temporarily stabilized, not permanently, although operator has 9 months after TD date to achieve restored status. No E&S issues observed at this time.
2229815 Primary Facility 2013-07-12 Drilling/Alteration No Violations Noted At the time of inspection, no E&S issues at the site. North East drilled to 650 feet at the time of inspection. Set conductor at 38 feet with 13 3/8 ft. Mine string (9 5/8”) set at 463 feet. Basket was set at 378 feet. 190 sacks of type 1 was used and cemented to surface. 8 hr. wait for cement time was recorded. Well was drilled to 650 feet. 7” section was set at 633 feet. 116 sacks of Type 1 were used with 8 bbl. of return. Well depth is anticipated to 2152 feet.
