Site ID sign on-site, appears to be in compliance with Act 9 standards. ESCGP-2, E&S/PCSM/Restoration plans, PPC plan, and ERP on-site. Well tag present, legible. Brine tank located in secondary containment unit, both appear sound. Access road gated, stabilized. Well pad stabilized. The areas surrounding sediment basin 1 are not yet permanently stabilized. These areas are temporarily stabilized with seed/mulch with minimal growth observed. The Department recommends the Operator continue to monitor and maintain these areas to ensure that >70% uniform and adequate vegetation is achieved for final permanent stabilization of the site. PCSM BMPs appear to be installed in accordance with the PCSM plan. The Department recommends that the Operator continue to monitor and maintain all BMPs as necessary in order to prevent any potential pollution events from occur

2613763 Primary Facility 2017-06-29 Routine/Complete Inspection No Violations Noted Marcellus. At time of inspection, well drilled and appears equipped for production. Well tag present, legible. Well surrounded by fence.

Following this inspection, the Department notified operator representative Matt Thomas of the above-noted observations via phone (Waste Management General). Mr. Thomas agreed to promptly investigate and follow-up with the Department. At 1405, Mr. Thomas notified the Department via phone that the staining was diesel fluid, most likely from a third party piece of equipment. All visibly impacted materials have been excavated in accordance with the Department's Spill Policy and will be disposed of properly. Per Mr. Thomas, the gravel in the cellar is wet due to water accumulation from recent rainfalls. Because the cellar is cemented, the water is unable to drain; there is no oil/petroleum product in the cellar. No violations are being noted at this time.
