2040833 Primary Facility 2012-02-02 Drilling/Alteration No Violations Noted First Class Energy was contracted by Penn View (Wayne Brannon ¿ tool pusher). Well sign and E&S plan were located at the access road. No freeboard issues were present at the site. Conductor pipe (13 3/8¿) was set at 42¿ (sanded in place). First Class drilled to 200 ft. and noted high water bearing zones at 80 ft., 120 ft. and 160 feet. High water bearing zones during drilling caused a semi- loss of flow of drilling cuttings and water through the diverter pipe to allow water/drill cuttings to flow on back side of conductor pipe causing a release around the rig. First Class stopped drilling and cemented the backside of the conductor pipe with 5.2 sks of Class A. Plug was down at 4:00 pm on 2/2/12 with no return. First Class was going to allow the 8 hrs. Cement time for set up of cement prior to drilling to 300 ft. to set a water protection casing. April Wieland and March Maestra were on-site and noted the release and other site issues.

Due to the potential of loss of circulation, cement basket was set at 42 feet. Pipe was cemented with 120 sacks of Class A with 8 bbls of return. Plug was down at 6:44 am on 2/3/12. Surface pipe (7¿) was set at 1,053 feet with 190 sacks of Class A with 5 bbls return. First Class Energy was currently drilling at 1, 570 feet. The anticipated total depth for the well was 4,640 feet.
