2024548 Primary Facility 2011-12-05 Drilling/Alteration No Violations Noted PC Rig #6 was contracted by Penn View (Jason Kline ¿ tool pusher). Well sign and E&S plan present at the access road. Filter fence is currently located topographically down gradient of site pad. No freeboard issues. During site inspection, no E&S or IW issues at the site. Conductor pipe (13 3/8¿) set at 40¿ (sanded in place). Water protection (9 5/8¿) was set at 211 ft. and cemented with 80 sks of Class A. Plug was down at 08:56 hours with 1 bbls return. Coal interval recorded at 117¿ ¿ 123¿. Water was recorded at 50feet. Intermediate casing (7¿) was set at 1,096 feet and cemented with 200 sks of Type I. Plug was down at 11:59 hours with 12 bbls of return. Phillips completed well to depth of 5000 ft. Wire line company started the logging of the well.
2026125 Primary Facility 2011-12-13 Routine/Complete Inspection No Violations Noted Well spudded 11/29/11. At the time of inspection, well Id sign, permit, and ES plan present. Access road ok, although department recommends additional rock be added to access road to ensure stability and prevent a possible pollution event. Well drilled, not yet equipped for production. One below grade pit located onsite with cuttings and fluid present. Recommended that operator properly dispose of pit as soon as possible, hence liquid frac appears frozen. Bmp's installed according to ES plan and appear to be adequately maintained. No accelerated erosion or sedimentation issues noted at the time of this inspection. Although it is recommended that ditches be rock lined as department observed laden (frozen) water along roadside. No violations noted at the time of this inspection, follow up inspection will be conducted in the near future.
2215332 Primary Facility 2013-10-21 Site Restoration No Violations Noted Well in production at time of inspection. Well tag legible. Brine tank appears to be sound. Dept. did not observe E&S issues and access road is in good condition. Vegetation growth is adequate and uniform and meets the minimum 70% coverage.
