During inspection accompanied by Brad Little of Penn View during inspection. Currently, well staked, construction has not begun due to proximity to nearest stream and pond. Pond previously filled by land owner, but recommend operator sustain 50ft buffer of disturbance from former pond location, as well as stream banks. Pits will be constructed 20in above seasonal high groundwater table and beyond 100ft from both the pond and stream locations. Operator plans to handle all drill cuttings onsite by encapsulation. If operator implements bmp¿s (hay bale reinforced filter fence) as outlined in ES plan and sustains minimum 50ft buffer(undisturbed areas) to stream and pond waiver shall be approved as reasonable efforts are being implemented to prevent possible pollution event to waters of the commonwealth. Additional GP¿s may be required in addition to OG-57 waiver which operator is responsible for obtaining.
1986010 Primary Facility 2011-06-16 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Penn view contracted PC Rig #1. Well sign and E&S plan posted on entrance road. Pit was a ligned with plastic. Conductor (13¿) pipe set to 30 ft. First Stringer (9 5/8¿) pipe set at 424 ft. with 175 sks Class A 3% with 9 bbl return. Filter fence present on the down gradient side of location. PC was waiting on the 8 hr time for requirement after cementing. No cement basket was used.

2003805 Primary Facility 2011-09-20 Compliance Evaluation No Violations Noted Site visit was follow-up to 7-13-11 inspection which resulted in an NOV. Mr. Brad Little attended the inspection.
2131672 Primary Facility 2012-12-11 Site Restoration No Violations Noted At time of inspection, well in production. Well tag present. Brine tank appeared to be sound. Access road in good condition. Previously existing areas of standing water near tree edge still observed (noted in previous inspection on 9/20/11; ID 2003805). Required 70% of uniform vegetative growth achieved. No E&S issues observed. Dept recommends that Operator removes temporary BMPs since permanent stabilization has been reached although Operator should continue to monitor and maintain site as needed.
