Spudded for 11/11/111. Pad and access road adequately rocked. No accelerated erosion or sedimenation issues noted. Bmp's installed and functioning according to plan, ditches and disturbed areas seeded/mulch although no growth observed yet. Reccomended that operator continue to monitor all bmp's until final/permanent stablization measurse have been achieved. One unlined below grade pit located onsite--noted water in pit. Operator shall remove water prior to lining and utilizing pit. Upon observation of ES plan, seasonal high groundwater table is reported to be greater than 60inches. Pits contents are to be diposed of onsite according to regulation, per disposal plan. No violations noted during this inspection.
2021591 Primary Facility 2011-11-22 Drilling/Alteration No Violations Noted PC Rig #5 was contracted by Penn View (Jason Kline ¿ tool pusher). Well sign and E&S plan present at the access road. Filter fence is currently located topographically down gradient of site pad. No freeboard issues. During site inspection, no E&S or IW issues at the site. Conductor pipe (20¿) set at 35¿ (sanded in place). Water protection (16¿) was set at 254 ft. and cemented with 160 sks of Class A. Plug was down at 10:46 hours with 13 bbls return. Open mine noted at 496 feet to 504 feet interval. Mine casing (9 5/8¿) was set at 546 feet with 200 sacks Type 1. Plug was down at 4:45 hours and grouted to surface. Water was recorded at 650feet. Intermediate casing (7¿) was set at 1,516 feet and cemented with 295 sks of Class A. Plug was down at 11/15/11 at 16:32 hours with 15 bbls of return. Coal was recorded at 345-348 feet, 397-403 feet interval and 496 ¿ 504 feet interval. Production casing (4 ½¿) was set to a depth of 5100 feet. Phillips moved to the Poorbaugh #1 site.

Appears site has not been uniformly regraded. Department recommends that Operator properly regrade the site to ensure compliance with final restoration requirements. A follow-up inspection will be conducted in the future to determine restoration compliance.
