2026941 Primary Facility 2011-12-13 Routine/Complete Inspection No Violations Noted At the time of inspection, well drilled and in production. Well spud 6/22/11. Shared access road ok. Brine tank appears structurally sound. No IW issues noted. Site not yet permanently restored (>70% uniform coverage), although seed/mulch observed. Recent earth disturbance observed on site, possibly from pipeline work. All disturbed areas shall be restored with 70% uniform vegetative cover within nine months from completion of drilling. Temporary bmp's in placing and functiong, operator shall continue to monitor and maintain all bmp's until final restoration achieved. No violations noted at the time of this inspection.
2184985 Primary Facility 2013-07-09 Site Restoration No Violations Noted Restr inspt. At time of inspection, well equipped for production. Well tag present. Brine tank sound, on gravel pad, level of contents appears to be low. Access road in good condition, culvert appears stable and functioning properly.
