Drilling completed 11/19/10. Well site has been restored adequate vegetative growth observed. Minor rutted noted on well pad due to truck traffic, recommend operator add additional rock to well pad in order to minimize the potential for a possible pollution event. Brine tank appears sound, no leaks or discharges noted, although observed possible evidence of brine release behind/downslope from brine tank. Operator shall ensure structural integrity is not compromised on tank and investigate the cause of accelerated ES and dead vegetation around and below brine tank. A follow up inspection will be conducted to ensure operator has addressed rutting concerns on well pad and structural integrity of brine tank is inspected. Department will also follow up with operator to determine what caused the deaddiscolored vegetation behind brine tank.
