1921671 Primary Facility 2010-10-22 Drilling/Alteration No Violations Noted According to records, permit present. Currently, PC rig #1 drilling currently at 1550 ft. Filter fence located topographically down gradient of site pad. Conductor pipe set at 36 ft. (13 5/8¿), 7¿ pipe run to 1222¿ with 1235 ft hole. No mine void present. Stratigraphy consisted of shale, sandstone, limestone, and coal seams on boring log. 7¿ pipe cemented with 160 sks applalite and 50 sks class a and plug down on 8:00 pm 10/21/10. Anticipated depth of 4350¿. Spud called on 10/18/10. Water at 75 ft, 175 ft and 240 ft. Gas zone at 1300 ft. Pit covered in 20 mil plastic. At the time of inspection, no E&S issues or IW issues. No freeboard issues. Water was being removed from pit for disposal.
1922821 Primary Facility 2010-10-27 Routine/Complete Inspection No Violations Noted During this inspection, well site properly signed with ES plan well recently drilled not yet equipped for production.Access road is muddy and observed mud on township road. Spoke with PV Rep Brad Little, requesting that roadway be clean and he assured me equipment was on the way to do so Observed that pit levels were at 2ft freeboard level, also suggested that operator pump below grade pit out to ensure that 2ft of lined freeboard is not exceeded, again Brad stated he would address that concern immediately as well. All temporary bmps¿ installed and properly functioning no accelerated erosion or sedimentation observed on well site. Recommend that operator add additional rock to access road and entrance to reduce concerns with mud. Observe that culvert pipes and outlets were installed and sufficiently rocked. Recommend operator continue to closely monitor access road and all temporary bmp¿s until final restoration actives have been completed to prevent any possible pollution events
1950723 Primary Facility 2011-02-04 Routine/Complete Inspection No Violations Noted At the time of inspection, well drilled and in production. Site Id sign and Es plan present. Brine tank ok, but noted minor staining on gravel below valve no leak detected, recommend operator ensure fitting/valve and tank are structurally stable to prevent a possible pollution event. Pits removed, but site not yet permanently restored. Temporary bmps in place, recommend operator continue to maintain all bmps until final permanent restoration is achieved. It is also recommended operator removed filter fence from culvert ditches, as this is not an approve use of the temporary bmp, although additional rock may be placed at culvert outlets. No accelerated erosion or sedimentation issues were noted. Site and access road snow covered.
2026144 Primary Facility 2011-12-13 Site Restoration No Violations Noted Well spudded 10/19/10. Drilling completed 10/26/10. Well drilled, equipped, and in production. Site restored with approximately 70% uniform vegetative cover. Temporary bmp's are in place, recommend operator remove in spring. Department noted staining (again) beneath and in front of brine tank from a possible release.
2185006 Primary Facility 2013-07-09 Routine/Complete Inspection No Violations Noted At time of inspt, well equipped for production.WT present. AR stable, runs next to ag field.Culverts appear to be functioning, sound. Final restoration achieved hence BMPs can be properly removed.Ruts developing near brine tank; Dept recommends Opr maintain to prevent any accel E&S pollution events from occurring.Previous inspt by WQS A. Weiland (record 1950723) on 2/04/2011 noted "minor staining on gravel below valve no leak detected". Following inspt by WQS A. Weiland (record 2026144) on 12/13/2011 noted "noted staining (again) beneath and in front of brine tank from a possible release" .At this time, staining still present; however, brine tank sound, no leaks detected.
2350345 Primary Facility 2015-03-06 Routine/Complete Inspection No Violations Noted At the time of inspection, no E&S issues were present at the site. Gas well was in production.
