MDS required by County to obtain building permit, hence well is located within conservation zone of Yellow Creek State Park. Site ID sign and ES plan onsite. Access rocked, although dept. recommended additional rock be added to roadway prior to drilling operations beginning. Operator is also responsible for ensuring township roadway is kept clean (free of mud and debris). Site is currently under construction. One (lined) below grade pit located onsite-department recommended that pit be re-liner to preserve structural integrity (free of wrinkles) ensure liner has not been compromised since cessation of tophole drilling. Operator has plans to haul all cuttings and production fluids to an approved facility. Bmps installed and appear to be properly functioning, no accelerated erosion or sedimentation issues noted on site or access road. Two sediment traps and diversion ditch are actively being constructed.

Access road stableoperator should continue to closely monitor and maintain access road entrance to ensure township road does not become muddy. One (lined) below grade drill pit located onsite¿adequate freeboard and security fence sustained. Recommend operator add additional rock to well pad, hence recent heavy rains have created muddy conditions on pad, although no accelerated erosion or sedimentation issues were noted. Bmps installed and appear to be properly functioning. No accelerated erosion or sedimentation issues noted offsite. Sediment traps, soil pile, and previously disturbed areas have been seeded and mulched¿although minimal growth observed. No industrial waste issues noted at time of inspection. Noted five closed/locked waste management boxes onsite- contain previously removed liner and waste from pit as a result of pit vandalism which occurred on 9-21-11: dept. requested that pit contents be sampled and all wastes be secured and removed to an approved facility.
