Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1911222 Primary Facility 2010-09-02 Drilling/Alteration No Violations Noted According to records, well permit granted on 5/14/10 to depth of 4830 ft. Well sign present. E&S controls were implemented. At the time of inspection, no E&S or IW issues. Pit aligned with 20 mil plastic and security fence. No freeboard issue. US Energy is contracted. Conductor pipe (13 7/8¿) set at 29 ft (sand). Water detected at 85¿ and 160¿. Stratigraphy consisted of coal, ss and sh. Surface casing (8 7/8) going to be set at 1374 ft. Depth of well was 1380 ft. No signs of gas leak from around well and equipment.
1911765 Primary Facility 2010-09-09 Drilling/Alteration No Violations Noted According to records, well permit granted on 5/14/10 to depth of 4830 ft. Well sign present. E&S controls were implemented. At the time of inspection, no E&S or IW issues. Pit aligned with 20 mil plastic and security fence. No freeboard issue. During inspection, water was transferred from pit to filter bag located in the wooded area. Called Kriebel to confirm the water was top hole water. Confirmed water by collecting a specific conductance reading in which meter which was calibrated showed a reading between 4 and 400 micoS per cm. (pre filter sample).
1914087 Primary Facility 2010-09-17 Routine/Complete Inspection No Violations Noted At the time of inspection, site properly signed with E&S plan onsite. Well drilled not yet equipped for production. One drill pit onsite. Pit sustained adequate freeboard and structurally sound. Pit appeared to have been recently pumped out as all fluids were removed. Noted the installation of filter fence down gradient from well head. Recommend that in the future operator secure pit liner by back filling, rather than single use of haybales, as this is not a recommended measure used to adequately secure pit liners. Observed that culvert outlets along access were rocked with large boulders, it is recommend that the ditches and outlets be dug slightly deeper and be lined with a smaller rock based material in order to properly function and minimize/prevent the threat of accelerated ES concerns.
1950653 Primary Facility 2011-02-04 Routine/Complete Inspection No Violations Noted At the time of inspection, site Id sign and Es plan onsite. Well drilled and in production. Brine tank surrounded by dikes appears sound. Access road and site snow covered. Pits removed and site regarded. Temporary bmp¿s in place, as permanent restoration has not yet been achieved, which is required within nine months after completion of drilling. No accelerated erosion or sedimentation issues noted in relation to site or access road.
1954081 Primary Facility 2011-03-02 Routine/Complete Inspection No Violations Noted Tag, tank, e&s ok.
2131681 Primary Facility 2012-12-11 Site Restoration No Violations Noted At time of inspection, well in production. Brine tank sorrounded by containment dike, both appeared sound. Minimal water in dike. Site permanently restored with >70% required vegetative growth. Access road in good condition. No E&S issues noted in relation to well site. Department recommends Operator continues to maintain site as needed.
2295857 Primary Facility 2014-05-07 Routine/Complete Inspection No Violations Noted At the time of inspection, no E&S issues were present. Gas well was in production.
2495616 Primary Facility 2016-06-21 Routine/Complete Inspection No Violations Noted no issues. system running
