1939435 Primary Facility 2010-12-21 Drilling/Alteration No Violations Noted Well sign present. E&S controls were implemented. At the time of inspection, no E&S or IW issues. Pit covered with 30 mil plastic and security fence. No freeboard issue. Falcon was contracted. Stratigraphy consisted of coal, ss and sh. Conductor pipe set at 42¿ (13 3/8¿ size). 1st stringer (9 5/8¿) was in the process of being set at 336 ft. Cement trucks should be present by the afternoon. Surface casing (7¿) going to 1500 ft. Well completed and anticipated depth going to 4900 ft.
1939896 Primary Facility 2010-12-20 Routine/Complete Inspection No Violations Noted At the time of this inspection, site properly signed with ES plan available. Rig onsite. Access road ok, but recommend operator re grade prior to final restoration. All temporary bmp¿s are installed and function according to ES plan, also recommend operator continue to monitor and maintain until spring thaw and final restoration activities have been completed. One below grade pit located on site, operator is responsible for ensuring that liner is structurally sound and free of fold and/or possible tears prior to uses. No freeboard issues, as during this inspection pit not yet utilized. Accompanied by WQS Marc Maestra.
1950659 Primary Facility 2011-02-04 Routine/Complete Inspection No Violations Noted At the time of inspection, well drilled and in production. Site Id sign and Es plan present. One brine tank on site surrounded by a dike exhibited a large dent in side, but could not detect a leak. Recommend that operator examine tank and it¿s structurally integrity to prevent a possible pollution event. Pits have been removed, site appears regarded, but permanent restoration not yet achieved; therefore, all temporary bmp¿s in place should remain until permanent final restoration achieved. No accelerated erosion or sedimentation issues noted. Site and access road snow covered.
2026116 Primary Facility 2011-12-13 Site Restoration No Violations Noted At the time of inspection, well drilled (spud 12/17/10 ;drilling completed 12/28/10), equipped, and in production. Well tag present. Well site permanently restored with adequate vegetation and rock. No accelerated erosion or sedimentation issues noted. One brine tank, appears structurally sound, located onsite and surrounded by vegetative dike. Access road stable. All temporary bmp's have been removed. No violations noted at time of this inspection.
