Pit aligned with 20 mil plastic. No freeboard issues. Conductor pipe set at 10¿ ¿ 30 ¿pipe and 30¿ ¿ 24¿ pipe and used 75 sks class A cement. Current depth at 60.¿ Permit and sedimentary log present. Water not recorded. Sedimentary consisted of ss/sh alternation and coal seam. Filter fence set around the site. Permit set for 9000 ft well to Marcellus.
1859413 Primary Facility 2010-01-28 Routine/Complete Inspection No Violations Noted Drill rig located onsite and drilling was ongoing at the time of the inspection. Observed three below grade pits located onsite and one was in use as a drill pit. This pit had more than the required two feet of lined freeboard. Erosion control measures had been implemented as outlined in the E&S plan located onsite. Observed no accelerated erosion & sedimentation in relation to the well-site. There was very little if any access road to speak of due to the proximity to Bethel Cemetary Road. Operator should continue to monitor the below grade pits and to insure the erosion control measures continue to function properly until well development and final restoration activities are completed.

1885591 Primary Facility 2010-05-18 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled not yet set for production. Well site was properly signed with E&S plan at site entrance. Noted three below grade pits were installed onsite and maintained more than a 2ft of lined freeboard, although it is recommended that operator continue to monitor freeboard levels at all times. Soil pile was temporarily seeded and mulched. Observed the properly installation/functioning of temporary bmp¿s as noted in erosion and sedimentation plan. No accelerated erosion or sedimentation concerns were noted in relation to well site or access road. Operator should continue to monitor all temporary E&S controls until final restoration activities have been completed and site is permanently stabilized. (Accompanied with, Water Quality Specialist, Greg Higgins during inspection).
1900994 Primary Facility 2010-07-27 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Well sign and E&S plan located at the entrance of site. Appears site has been recently seeded and mulched as<70% uniform vegetative growth is observed. Four tanks on site are surrounded by dike. No accelerated erosion or sedimentation issues observed. No H2S odors detected by operator during drilling or fracing operations. Tempoary bmp¿s installed as outlined in plan and functioning. Upon final restoration (permanent stabilization, >70% uniform vegetative growth) operator may remove all temporary bmps, until they all should be closely monitored. Access road entrance stable.
1939395 Primary Facility 2010-12-21 Routine/Complete Inspection No Violations Noted At the time of inspection, no E&S or IW issues at the site. Well tag present. Site has been restored.
