Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1856194 Primary Facility 2010-01-14 Routine/Complete Inspection No Violations Noted Drill rig located onsite at the time of the inspection. Below grade pit located onsite and it had more than the two feet of required freeboard. Erosion control measures implemented as outlined in the E&S plan located onsite. No accelerated erosion & sedimentation was observed onsite or with respect to the access road at the time of the inspection. Operator should continue to monitor the site until well development and final site restoration activities are completed.
1856365 Primary Facility 2010-01-14 Drilling/Alteration No Violations Noted According to records, well permit granted on 12/18/09 to depth of 4900 ft. Spud called for 1/07/10. Well sign and E&S plan present at the entrance of site pad. Site pad was completed. E&S controls were implemented. Drilling company set conductor at 38¿ (13 3/8¿) sanded. First Stringer set at 1179¿ (7¿) at 230 sks class A, 120 sks berrasel and 500 sks gel with return. Current depth was at 4998 ft. Water recorded on log at 130 ft. Pit aligned with 20 mil and no freeboard issues. At the time of inspection, no E&S or IW issues
1886499 Primary Facility 2010-05-18 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled not yet set into production. E&S controls were installed/functioning according to E&S plan on site; therefore, no accelerated E&S issues were noted in relation to access road or well site. One below grade pit was observed onsite which maintained more than the required 2ft of lined freeboard. It is recommended that operator continue to closely monitor all E&S controls until final restoration activities have been completed.
1914141 Primary Facility 2010-09-17 Routine/Complete Inspection No Violations Noted During this inspection, the well site was properly signed with E&S plan available. Well drilled not yet equipped for production. One below grade pit observed onsite containing drill cuttings and some liquid fraction. Noted brush/grub line installed and filter fence below grade of well site. Operator should also be advised that well site is to be permanently restored, minimum 70% vegetative cover or other permanent stabilization measure that can resist erosion, within nine months of completion of drilling. Dept unable to located official WRAC report, recommend that operator resubmit to DEP. At this time site is not restored, nor temporarily seeded or mulched. No accelerated ES issues were noted in relation to well site. It is recommended that operator continue to monitor and maintain all bmps until permanent restoration measure are achieved. A follow up inspection will be conducted to ensure final restoration is complete.
1941817 Primary Facility 2011-01-10 Routine/Complete Inspection No Violations Noted During this inspection, site properly signed with ES plan. Well drilled not yet equipped for production. All bmp¿s installed/functioning according to plan. Appears that site has been reclaimed, below grade pit removed and site re-grade, seeded and mulched. Well site snow covered; therefore, it is difficult to determine if 70% uniform vegetative cover is achieved. Operator may need to reseed in mulch in spring after thaw to ensure adequate uniform vegetative growth is observed, until then operator should continue to maintain all bmp¿s No accelerated erosion or sedimentation issues noted. Follow up inspection in spring will be conducted to confirm final restoration. Accompanied with WQS Marc Maestra.
1978919 Primary Facility 2011-05-23 Site Restoration Violation(s) Noted During this inspection, site properly signed with ES plan. Well tag present. Drilling completed 1/14/2010. Access road is not adequately maintained/ stabilized, obvious accelerated erosion and sedimentation issues observed. Site has not been adequately restored as >70% vegetative growth observed nor adequately re-graded. Observed significant ponding of water and erosion (unlined) channels surrounding site. Brine tank onsite ¾ full also observed minor ponding of water surrounding tank, although no dead vegetation observed during this inspection. Dept is also requesting information on final pit disposal method and manifests on how/where liquid fraction from pit was disposed of.
1981818 Primary Facility 2011-06-20 Routine/Complete Inspection No Violations Noted Id sign and e&s plan onsite. Drilling complete. Well equipped for production. Tank ok. Site graded and vegetation started. No open pits.
1997797 Primary Facility 2011-08-17 Drilling/Alteration No Violations Noted According to records, well permit granted on 12/18/09 to depth of 4900 ft. on 1/14/10. At the time of inspection, no E&S or IW issues. Gas well was in production. Brine tank contained 50 bbls fluid in a 100 bbls tank. Wellhead had annulus open and separator at 300 psi. Data logger showed 350 psi/ 25 psi.
2012876 Primary Facility 2011-10-19 Compliance Evaluation No Violations Noted CEI Inspection for failure to restore. Site restored no violations noted during this inspection. Check Brine tank structurally integrity.
2026853 Primary Facility 2011-12-13 Routine/Complete Inspection No Violations Noted At the time of inspection, no E&S or IW issues. Brine tank contained 60 bbls fluid in a 100 bbls tank. Wellhead had annulus open. Compressor was added at the site. Meter showed a pressure reading of 350 psi/ 0 psi. Well tag present.
2173353 Primary Facility 2013-05-21 Routine/Complete Inspection No Violations Noted Site adjacent to compressor station. At time of inspection, well in production. Well tag present. Brine tank sound, appears to be one-third full of fluids. "Shared" entrance rocked, in good condition. No accelerated erosion or sedimentation issues observed.
2234800 Primary Facility 2014-01-08 Routine/Complete Inspection No Violations Noted Well equipped for production at time of inspection. Brine tank appears to be sound. Compressor unit on site. Access road is in good condition. Site is covered in snow making evaluation of vegetative growth and E&S controls difficult. Well site will be reassessed when weather conditions improve.
