Well site was identified with well sign and E&S plan. No IW and E&S issues at the time of inspection. Pit aligned with 20 mil plastic and had no freeboard issues. Filter fence located topographically down gradient of site pad. Conductor pipe set at 42.55 ft. (13 3/8¿), 9 5/8¿ pipe set at 188.90 ft with 125 sks class A. Stratigraphy consisted of shale, sandstone, limestone, and coal seams on boring log. Permit present. Anticipated depth of 4700¿
1858013 Primary Facility 2010-01-22 Routine/Complete Inspection No Violations Noted Drilling had been completed at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of freeboard. Observed mud and sediment being transported out onto State Route 259 due to the drill rig being moved off-site at the time of the inspection. Contacted operator representative Brad Little who stated that a broom and plow truck where enroute to the site. WQS Higgins stated that after the mud is cleared from the road it should be collected and taken back up onsite, and the access road should be rerocked before any further well development activities are undertaken. Operator representative Brad Little agreed. The operator should monitor all erosion control BMP's until final restoration activities are completed.
1900100 Primary Facility 2010-07-21 Site Restoration No Violations Noted At the time of inspection, well was properly tagged, drilled, and in production. Site was adequately and uniformly restored with at least 70% vegetative cover. No accelerated erosion or sedimentation issues were noted in relation to access road or well site. Brine tank appeared sound. Now IW issues noted. Access road stable. Based on prior inspection reports, drilling was completed January 2010, but department has not yet received official Well Record and Complete Report.
