1870943 Primary Facility 2010-03-18 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Pit aligned with 20 mil plastic and no freeboard issues. Falcon set conductor pipe (13 3/8¿) 42 `. 7¿ pipe set at 1308¿ used 270 sks Class A with 0 bbl return. 8 hr set up time recorded on log. Falcon was at 3660¿. Water detected at 210¿, 240¿, 500¿ and 520¿. Spud called on 3/12/10. Well permit present along with well log. Stratigraphy consisted of sandstone, limestone, shale and coal. Well sign and E&S plan located at the entrance way of site. Down gradient filter fence present along site pad.
1872019 Primary Facility 2010-03-16 Routine/Complete Inspection No Violations Noted Drilling was appeared to have been complelted at the time of the inspection. Below grade pit located onsite and te liquid fraction had been removed from the pit, it had been relined and was in the process of being filled for well stimulation. Observed no accelerated erosion and sedimentation on the access road or well-site.
1954019 Primary Facility 2011-03-02 Site Restoration No Violations Noted At the time of inspection, well tag present. Well in production. Well completed on 03/18/10. Site adequately and uniformly restored, hence operator may remove all temporary bmp¿s previously installed. No accelerated erosion or sedimentation issues were noted in relation to well site or access road. Access road adequately rocked. Brine tank appears sound. Well site gated.
