Well site was identified with well sign and E&S plan. E&S concerns at the time of inspection with drainage pipe. Rip rap required around inlet and outlet of pipe requested that stone added to these areas. No IW discharge issues. Pit aligned with 20 mil plastic and had no freeboard issues. Filter fence located topographically down gradient of site pad.
1833189 Primary Facility 2009-09-24 Routine/Complete Inspection No Violations Noted At the time of inspection, well was drilled and proper signage and E&S plan were on site. Access road was in stable condition. Pit sustained 2ft freeboard and appeared to be in good condition. BMP's of pipe culverts with rock outlets and filter fabric fence were installed and functioning properly to limit erosion and sedimentation in relation to the site and access road.

Site has not been restored within the 9 month period of completion of well. Drilling pit still remains open. Security fence has fallen over around pit. Excavated pit area for shale fill still remains open. Well tag not placed on well. E&S plan and well sign has been removed from site. Well is not in production. Filter fence is present on the topographic down gradient side of site pad. At the time of inspection, no E&S or IW issues.
1898223 Primary Facility 2010-07-15 Compliance Evaluation No Violations Noted Based on well completion record, well was completed to depth of 5100 ft on 9/17/09. Site has been restored. Pits (drilling and fill material) were filled in. Site has been graded and grass and seeded. Site will be required monitoring until proper amount of surface growth has been restored. Well tag placed on well. E&S plan and well sign located at the entrance of property. Well is not in production. As per my conversation with McIntire, surface access being pursued for transmission line. Well is expected to be on-line within 12 months of completion. Filter fence is present on the topographic down gradient side of site pad. At the time of inspection, no E&S or IW issues.
1914039 Primary Facility 2010-09-17 Site Restoration No Violations Noted At the time of inspection, well was drilled and properly tagged. Drilling completed on 09/17/09. Site adequately and uniformly restored with minimum 70% vegetative cover. Pits removed. Note: Inspector Paul Wyrwas issued a violation for failure to restore within nine months of drilling on 06/30/10 inspection. No accelerated erosion or sedimentation issues were noted with site or access road during this inspection. Recommend that operator remove filter fence, since adequate permanent restoration has been achieved. All utilized permanent bmps appeared to be installed and functioning properly (ie culvert ditches, rock outlets).
